Gone are the days that patients are cared for by a single “doctor”. Today, in modern healthcare systems, patients are seen, evaluated, and treated by groups of providers. This innovative “team approach” has a number of advantages. It allows for multiple specialists to evaluate the same patient, and synergize their training, talents, and insights for optimal patient outcome. It also allows healthcare providers to rapidly and efficiently onboard new techniques and technologies, as learning these new innovations is done as a team, rather than as an individualized event. It allows for communication, collaboration, and growth amongst specialties as we treat and elevate our patients. This new standard is without question here to stay, and as we continue to evolve, the team approach to healthcare will undoubtedly strengthen. The promise and potential for this healthcare future is incredibly bright, but like many new innovations, there can be issues. Positive growth rarely occurs in a straight line, and with every advance, there are new issues, new concerns. From my perspective, the biggest concern with the new model of medicine is the loss of the provider-patient relationship. As patients are continually treated by groups of providers rather than individuals, it becomes easy for doctors and nurses to hyperfocus on the task at hand, rather than focus on the bigger picture. **Too often, we as medical providers treat a disease process rather than the human with the disease process.** As we continue to increase the complexity of healthcare, it becomes easy to lose the most important concept in healthcare: the human concept. Ocean Drive Dermatology was founded in 2019 to combat the loss of humanity in the care of your skin. We are not here to create an “assembly line” experience. We are not here to reduce you to your diagnosis, or to treat you within a template. Instead, we are here to provide you and your family a human experience based upon listening and learning about you, the patient. Our appointments are a minimum of thirty minutes in length, whether it be for a single spot of concern, or establishment of comprehensive care. We offer the entire spectrum of modern Dermatology and Plastic Surgery, encompassed in a comfortable, and hopefully pleasant, experience. Lastly, we are dedicated to offering the entire spectrum of services needed for patients. We do not offer just Botox, or Fillers. We offer the entire portfolio of both neuromodulators (**Botox, Xeomin, Dysport, Daxy**), as well as all of the major safe, effective, FDA-approved filler platforms (**Radiesse, Restylane, Juvederm, Versa, Renuva, Belotero, etc.**). Also, we offer cosmetic laser resurfacing including full field erbium ablative, fractional, broadband light/IPL, and Halo. For our medical dermatology patients, we offer all of our cancer patients the benefits of Mohs surgery without any of its limitations. Our on site [dermatopathology](https://www.surgicalpathlabs.com/) services ensures that the highest standards for skin cancer care are followed on every case. For more advanced issues, on site referrals are made to Plastic Surgery the same day. Further, all patients undergoing cancer resections at Ocean Drive can expect to have the finest possible closure provided BEFORE they leave the office. Finally, I proudly serves as the Chairman of the Division of Plastic Surgery at Cleveland Clinic, and as the Vice Chairman of the Department of Surgery. We routinely provide advanced resections requiring anesthesia support locally at the Cleveland Clinic. We are very proud of our facility, and more importantly, of our providers, staff, and patients. We are eager to meet and serve you. I personally hope to see you soon. #### Alan J. Firstly, Nick grew up in Pennsylvania, where he graduated from Riverview High School in Oakmont, Pennsylvania. As an athlete and a scholar, Nick flourished in both mathematics and biological sciences, and was a member of the Drama Club and Key Club. He was also Captain of his Varsity Golf Team. After High School, he attended Robert Morris University, majoring in Mathematics and Computer Science. Nick graduated magna cum laude with a Masters degree in Information Systems Management. While excelling in the digital industry, Nick felt that his potential was incompletely realized in this field, and he sought to have a more relationship based career. **Therefore, he decided to follow in the footsteps of his grandfather who practiced medicine for over 50 years as a family physician.** #### Professional Education & Experience For these reasons, Nick Woltjen enrolled in the Physician Assistant program at Chatham University in Pittsburgh, Pennsylvania where he earned his second Masters degree in physician assistant studies. It was during this time that Nick developed a deep interest for treating skin cancer and skin disorders. After graduating in the top of his class, Nick Woltjen was offered a position as a Dermatologic Physician Assistant at the University of Pittsburgh Medical Center (UPMC). During his tenure at UPMC, Nick functioned independently as a comprehensive provider of dermatologic services. He routinely collaborated with thought leaders in Dermatology and Dermatologic Surgery at UPMC, including Dr. Suzan Obagi. He also actively participated in the training and development of Physician Assistants and Dermatologists training at the University of Pittsburgh. - Physician Assistant, Dermatology, University of Pittsburgh Medical Center, Pittsburgh, PA - MPAS, Chatham University, Pittsburgh, PA - MS., Robert Morris University, Moon Township, Pennsylvania - BS., Robert Morris University, Moon Township, Pennsylvania, Magna Cum Laude #### Nick’sDermatology Expertise Nick Woltjen’s unique skill set allows him to comprehensively manage complex acute and chronic skin disorders. His passions include skin cancer evaluations, prevention of skin cancers, and surgical and medical treatments of skin cancers. Also, he has a remarkably vast experience with management of premalignant lesions, rashes, psoriasis, and lupus. Lastly, Nick provides comprehensive nonsurgical therapy for hair loss, including laser therapy, and PRP injections for alopecia. Most noteworthy, Nick’s approach to Dermatology starts with establishing a relationship with the patients he is privileged to meet. The relationship begins with a friendly conversation, not a scalpel. After discussing patient needs and concerns, a comprehensive plan is formulated with the patient, not just for the patient. Furthermore, Nick provides surgical management of skin cancers and disease for the trunk, arms and legs. For more aesthetically sensitive areas, including the face, breast, and hands, Nick and Dr. Durkin work together to provide the best possible synergy between cancer care and aesthetic outcome. Above all, Nick and Dr. Durkin both pride themselves on collaboration, transparency, and educating our patients in all options. They work together on complex cases to offer comprehensive dermatology and plastic surgery services to our patients. Likewise, our patients enjoy the added convenience of being able to see both providers in the same office. --- ## PhilanthropyOcean Drive Dermatology **Link:** [PhilanthropyOcean Drive Dermatology](https://www.oceandrivedermatology.com/about/ocean-drive-dermatology-philanthropy/) ([md](https://www.oceandrivedermatology.com/about/ocean-drive-dermatology-philanthropy/index.html.md)) **Description:** Ocean Drive Dermatology is honored to give back to the Vero Beach and surrounding communities. ### Content #### Philanthropy Ocean Drive Dermatology is honored to give back to the Vero Beach and surrounding communities. **Below are some examples of the philanthropic work that Ocean Drive Dermatology has participated in since its grand opening in 2020.** --- ## Our TeamOcean Drive Dermatology **Link:** [Our TeamOcean Drive Dermatology](https://www.oceandrivedermatology.com/about/our-team/) ([md](https://www.oceandrivedermatology.com/about/our-team/index.html.md)) **Description:** Learn more about the team behind Ocean Drive Dermatology in Vero Beach. ### Content #### About Us - Dermatology We are not a regular dermatology practice. Call us today to schedule a consultation, and get your full head of hair back. ### Content **Rogaine** is a topical liquid or foam preparation in either the 2% or 5% strengths that is used for hair loss. Typically, I recommend the 5% version for both male and female hair loss for our **Vero Beach hair restoration** patients. It will take a minimum of 6 months to determine efficacy of this medication. Typically, it is used twice a day. In the beginning of treatment some patients notice an increase in shedding and this is considered normal. The foam is typically easier to apply and less messy, but some patients prefer the liquid. Liquid is typically less expensive. **Just like Propecia, any benefit during treatment is lost over time if discontinued.** --- ## Skin CareOcean Drive Dermatology **Link:** [Skin CareOcean Drive Dermatology](https://www.oceandrivedermatology.com/products/skin-care-advice/) ([md](https://www.oceandrivedermatology.com/products/skin-care-advice/index.html.md)) **Description:** Ocean Drive Dermatology in collaboration with Ocean Drive MedSpa is happy to offer skin care guidance to our patients. ### Content #### Skin Care Guidance Ocean Drive Dermatology in collaboration with Ocean Drive MedSpa is happy to offer **skin care guidance** to our patients. We can help demystify what ingredients to use and which ones to avoid. In a social media driven world it is sometimes hard to determine what is truly a breakthrough product or ingredient and what is just clever marketing. Our **expert Vero Beach aestheticians** and skin care experts will help review your current products and even recommend other products that may be beneficial. No matter how many [injectables](/injectables-fillers/) or [lasers](/laser-treatments/) a person does, the foundation for great skin is in a *consistent skincare routine* with excellent products and ingredients. The foundation includes wearing sunscreen daily. There are two major sunscreen types: physical and chemical. Although we prefer physical based sunscreens that include zinc oxide and titanium dioxide, in reality any sunscreen is better than none. Find one that feels good on your skin and you are willing to reapply throughout the day when appropriate. Try to utilize an SPF of at least 30 with a recommendation of SPF 50. SPF 30 provides about 93% UV protection while SPF 50 is over 97%. **Don’t forget to add in your hat for the best protection and avoid the intense UV rays between 10 am and 2 pm if possible.** #### Moisturizers Moisturizers are another important component of a **good skin care routine**. One of our favorite ingredients are ceramides. These fats or lipids make up a portion of the top layer of skin called the epidermis. They help lock in moisture, but damage from sun exposure and simply age cause the barrier to degrade overtime. Another important ingredient in moisturizers is hyaluronic acid. Although the word acid may have you thinking this is going to dry the skin out it actually helps lock in the moisturizer along with reducing some signs of aging. #### Vitamin C Another heavy hitter in the skincare routine is Vitamin C. This is an absolute must if your skin tolerates Vitamin C serums or creams. Vitamin C is a powerful antioxidant that helps brighten the skin, lighten dark spots, and can also hydrate the skin. If you want another reason to utilize a good Vitamin C serum daily, it also helps promote collagen! Sunscreen, moisturizers, retinols and vitamin C are the building blocks of a good skin care routine. The next step is to schedule a consultation with one of our Vero Beach **skin care experts and aestheticians** to create a routine that you can follow on a daily basis. They will help guide your product selection based on your goals and your skin type. --- ## Insurance and FinancingOcean Drive Dermatology **Link:** [Insurance and FinancingOcean Drive Dermatology](https://www.oceandrivedermatology.com/resources/insurance-financing/) ([md](https://www.oceandrivedermatology.com/resources/insurance-financing/index.html.md)) **Description:** We provide affordable self-pay prices for office and telemedicine consultations.Ocean Drive Dermatology offers Care Credit and Alphaeon financing for cash-priced fillers, neurotoxins, non-surgical body contouring, and laser treatments. So, when Sarah Eggenberger was recently diagnosed with an aggressive form of skin cancer (morpheaform basal cell carcinoma) right on her face, the stakes couldn't have been higher. She didn't just need the cancer gone; she needed a surgical team that could guarantee complete removal without leaving a disfiguring scar that might end her on-camera career. To ensure the best possible outcome, Sarah flew into Vero Beach to trust her care to Dr. Alan Durkin and the team at Ocean Drive Dermatology. We are thrilled to report that she is now cancer-free with a beautiful, scarless result. **[.title-xs .text-center]Sharing the Journey on Good Day LA[]** Sarah’s journey was so remarkable that she recently invited Dr. Durkin to join her in Los Angeles on the set of FOX 11’s *Good Day LA*. They shared her story with millions of viewers to raise awareness about skin cancer detection and the importance of choosing the right surgeon for facial procedures. **[.title-xs .text-center]The Ocean Drive Difference: Aesthetics Meets Oncology[]** In the segment above, Sarah opens up about the initial shock of her diagnosis—having initially mistaken the cancer for a simple age spot and the very real fear of what surgical removal might look like. For Dr. Durkin, Sarah’s case perfectly highlights the core philosophy of our practice: treating skin cancer shouldn't just be about removing the tumor; it must be about restoring the patient. "The goal with basal and squamous cell carcinomas is your *best* result, not just removing the least amount of tissue," Dr. Durkin explains. Many patients worry they have to choose between being cancer-free and looking like themselves. At Ocean Drive Dermatology, we bridge that gap. By harmonizing the meticulous principles of oncologic cancer removal with the refined techniques of aesthetic plastic surgery, we strive to ensure every patient walks away with an uncompromised aesthetic outcome. **[.title-xs .text-center]A Flawless Result[]** Today, Sarah has returned to her work in front of the national camera with confidence. We are incredibly grateful to her for using her vast platform to share this vital message: you do not have to compromise your appearance to cure your skin cancer. *Have you had your annual skin check? This is because there is a higher likelihood of spreading the disease throughout the body. ### Content There are three major skin cancer types. **Basal cell carcinoma**, **squamous cell carcinoma** and **malignant melanoma**. Of these three, malignant melanoma is considered the most serious form of skin cancer. This is because there is a higher likelihood of metastasis or spreading of disease throughout the body. In general, early stage melanomas such as stage 1 have good outcomes. Later stages, such as stage 4 are more likely to have metastasis. There are four major types of malignant melanoma including superficial spreading, nodular melanoma, lentigo maligna melanoma, and acral (hands, feet, ears, nose) melanoma. The most common of the four types of malignant melanoma is superficial spreading. Upwards of 70 percent of all melanomas fall into this category and that tends to be a good thing. The majority of superficial spreading melanomas are exactly that; superficial. This tends to mean the skin cancer is thinner or the tumor thickness is less than 1 mm. Tumor thickness is extremely important when diagnosed with melanoma as the thicker the tumor the harder it is to cure and the higher chances of metastasis. With superficial spreading melanoma the majority of cases are curable. Next up is nodular melanoma and this is the second most common type of melanoma after superficial spreading. Compared to superficial spreading melanoma this type of tumor tends to be thicker. Most nodular melanoma are twice the depth of superficial spreading melanomas, 2 mm. That doesn’t seem like a big deal, but the skin is not very thick. So, 2 mm is actually quite deep. Lentigo maligna melanoma we tend to see in older patients in areas on skin that has had long term sun exposure. This means areas like the face and chest. These tend to be slow growing tumors that may develop over the course of years when compared to other melanomas that may seem to appear out of nowhere. Lastly, acral melanoma, those that tend to appear on the peripheral areas of our body is the least common. As compared to fair skinned individuals, this type tends to affect those with darker complexions. These are typically found on the hands and feet or even in the nailbeds. Cumulative sun exposure from the beach, boat, tanning beds, etc. is the most obvious risk factor for developing a **melanoma**. However, there are numerous other factors that may contribute to the development of this deadly skin cancer. Fair skin, light eyes, having numerous dark moles on your body, family history of melanoma, and even being immunosuppressed by certain medications or disease processes. Keep in mind that **although existing moles can transform into melanoma the vast majority of melanomas are new spots**. So always be on the lookout for the “ugly duckling”, which is the new mole that does not look like any other mole on your body! **Melanoma** is not always easy to identify in its early stages. The best thing to do is once per month look over your skin, preferably with the help of someone for areas that are hard to observe such as your back. Follow the ABCDE rule and if you notice something that doesn’t look right make an appointment to be seen as soon as possible. **A** stands for asymmetry. This simply means that if you draw a line down the middle of the mole the two halves are not identical. **B** stands or border irregularity. You are looking for borders that are jagged or do not look like your other moles. **C** stand for color changes. Do you have dozens or brown or black moles, but one looks like it has other colors in it like red, blue, or even white? Those are the types of color changes you should be on the lookout for. **D** stands for diameter. Although not a hard and fast rule, the majority of melanomas tend to be 6 mm or larger. That’s about the size of a pencil eraser. **E** stands for evolving or evolution. In my opinion this is the most important. I like to call it “ever changing”. If you notice a mole that is changing size, shape, color, texture, is bleeding, or just does not “look right”; it’s time to be seen in the office. **Treatment for melanomas** include surgical excision with wide safety margins. In addition to surgery some patient may need to undergo further treatment with chemotherapy, radiation, immunotherapy or even a combination of therapies. Tumor staging is one of the most important prognostic factors when being diagnosed with malignant melanoma. Although all cases of melanoma and patients are unique, this gives a general guideline of what to expect in regards to outcomes. Melanoma has four stages where the higher the staging the worse the prognosis. Depending on the stage of melanoma and the tumor thickness additional tests may be ordered. For example, a tumor that is 1.1 mm thick would require a sentinel lymph node biopsy and PET/CT scans. This is to help determine if the tumor has spread to other parts of the body. Thankfully, melanoma research and treatments are getting better and better. The five-year survival rate for melanoma overall is around 91%; and those with localized disease (not spreading) can be as high as 98%. The American Cancer Society provides the following information regarding survival rates for melanoma by stages. Please remember that all patients and all tumors are unique. These are overall and generalized survival rates. **Stage IA:** The 5-year survival rate is around 97%. The 10-year survival is around 95%. **Stage IB:** The 5-year survival rate is around 92%. The 10-year survival is around 86%. **Stage IIA:** The 5-year survival rate is around 81%. The 10-year survival is around 67%. **Stage IIB:** The 5-year survival rate is around 70%. The 10-year survival is around 57%. **Stage IIC:** The 5-year survival rate is around 53%. The 10-year survival is around 40%. **Stage IIIA:** The 5-year survival rate is around 78%. The 10-year survival is around 68%.* **Stage IIIB:** The 5-year survival rate is around 59%. The 10-year survival is around 43%. **Stage IIIC:** The 5-year survival rate is around 40%. The 10-year survival is around 24%. **Stage IV:** The 5-year survival rate is about 15% to 20%. The 10-year survival is about 10% to 15%. The outlook is better if the spread is only to distant parts of the skin or distant lymph nodes rather than to other organs, and if the blood level of lactate dehydrogenase (LDH) is normal *The survival rate is higher for stage IIIA cancers than for some stage II cancers. This is likely because the main (primary) tumor is often less advanced for IIIA cancers, although this is not clear. --- ## Melasma **Link:** [Melasma](https://www.oceandrivedermatology.com/skin-conditions/melasma/) ([md](https://www.oceandrivedermatology.com/skin-conditions/melasma/index.html.md)) **Type:** Service **Description:** Melasma is a skin condition that causes darkening of the skin, usually on the face. Contact Ocean Drive Dermatology today to find out more about treating Melasma. ### Content #### Where do these dark spots on my face come from? **Melasma** is a skin condition that causes darkening the skin, usually on the face. There could be one little area of brown on the upper lip, or the entire forehead, cheeks, and chin are affected to varying degrees. The color tends to be light to dark brown and usually worsens with excess sun exposure. Although most patients with melasma are female; it is possible for a man to have it. Sometimes these brown spots appear during pregnancy, after starting birth control pills, and even with certain medications. The exact cause of melasma is not known. *Hormones may cause a role in Melasma*, but it is more complex than that. Melasma tends to be chronic. This means treatment is ongoing and I can’t offer a “cure” for it. #### Treatment Options **Treatment for melasma** usually starts with topical therapies. Most patients start with over the counter bleaching or fade creams. I typically have a treatment protocol that starts with prescription medications. For some of our Vero Beach **Melasma** patients, I will work with a compounding pharmacy to create a specialized prescription that includes multiple medicines that can help treat it. If topical therapy does not work then the next step tends to be laser therapy. Laser can include either non-ablative treatments such as [Broadband Light (BBL)](/laser-treatments/broadband-light-bbl/) or [ablative treatments such a Fractional](/laser-treatments/ablative-vs-non-ablative-lasers/) or [Halo Laser](/laser-treatments/halo-hybrid-fractional-laser/). Studies show that laser can be beneficial to melasma, neutral to melasma, and in some cases may make it worse. #### Prevention **All Melasma patients must adhere to a strict sun protection routine**. This includes sunscreen, hats, and finding shade. Makeup that has sunscreen in it typically is not enough. I have seen numerous times where a patient will have their melasma well controlled and then come back from a beach vacation and notice a few weeks later it is flaring up. --- ## Injectables Fillers **Link:** [Injectables Fillers](https://www.oceandrivedermatology.com/injectables-fillers/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/index.html.md)) **Type:** Service **Description:** Ocean Drive Dermatology in Vero Beach offers a full range of injectable cosmetic treatments and dermal fillers, including Botox, Dysport, Xeomin, and more. ### Content --- ## Laser Treatments **Link:** [Laser Treatments](https://www.oceandrivedermatology.com/laser-treatments/) ([md](https://www.oceandrivedermatology.com/laser-treatments/index.html.md)) **Type:** Service **Description:** At Ocean Drive Dermatology in Vero Beach, FL, we offer a range of laser treatments including Ablative laser, BBL Hero, Broad Band Light, Halo laser, and more. ### Content --- ## Skin Conditions **Link:** [Skin Conditions](https://www.oceandrivedermatology.com/skin-conditions/) ([md](https://www.oceandrivedermatology.com/skin-conditions/index.html.md)) **Type:** Service **Description:** At Ocean Drive Dermatology in Vero Beach, we offer treatments for skin conditions of all kinds. Rarer than melanoma. However, every year we see these come into our office. ### Content #### What is Merkel Cell Carcinoma? **Merkel Cell Carcinoma** is another form of skin cancer that tends to be even more aggressive than melanoma and sometimes referred to as a neuroendocrine carcinoma. *Merkel Cell Carcinoma is rare*. Rarer than melanoma. However, every year we see these come into our office. We are not certain exactly what causes a Merkel Cell Carcinoma to appear. There is some evidence that sun exposure plays a role as most of these carcinomas appear on the head and neck in sun exposed areas. Also, there is more recent evidence that shows there might be a virus that lives on our skin that plays a role in the development of this carcinoma. Bottom line is we are not sure exactly how and why it develops. #### Who is at risk to get Merkel Cell Carcinoma? Technically, anyone could develop a **Merkel Cell Carcinoma**. There are certain risk factors that seem to be more prevalent in those that develop this carcinoma. Excess UV exposure throughout your lifetime including tanning bed use, history of previous skin cancers such as squamous and basal cell carcinomas, fairer skin types, and generally those with a weakened immune system from something like organ transplants or even HIV. #### How do I recognize a Merkel Cell Carcinoma? Just like any skin cancer, you are looking for something new to appear on your skin or something to be changing on your skin. **Merkel Cell Carcinoma tends to be red or violaceous in color**. This type of carcinoma tends to enlarge over time, and just like other skin cancers it can bleed. If you are looking over your skin once a month for new or changing lesions you might recognize a new or changing spot. This is the best way to spot it along with yearly skin cancer screenings in the office. If a suspicious spot is found during your skin cancer screening it will be biopsied. Stages of Merkel Cell Carcinoma There are three stages of Merkel Cell Carcinoma. There is no agreed upon staging for this type of carcinoma, but one of the most common and widely used is the system suggested by Yiengpruksawan et al, which uses three stages. *About 55% of patients have stage 1 disease when first diagnosed.* **Stage I** – Absence of lymphadenopathy **Stage IA** – Tumors < 2 cm **Stage IB** – Tumors > 2 cm *About 31% of patients have stage 2 disease when first diagnosed.* **Stage II** – Positive regional lymphadenopathy *About 6% of patients have stage 3 disease when first diagnosed and tends to carry a very poor prognosis.* **Stage III** – Evidence of distant metastases #### Treatment Options After confirmation that you have a **Merkel Cell Carcinoma** there are several steps that need to occur. The first step may be to obtain CT or PET imaging of your entire body looking for metastases as this carcinoma tends to spread followed by wide local excision of the carcinoma. Wide local excision meaning possibly 1 to 3 cm of clear margins. This is larger than some melanoma excisions. If there happens to be lymph node involvement the involved lymph nodes are typically removed during surgery. Depending on the staging of the carcinoma and whether there are metastases, it may be necessary to work with an oncologist. The oncologist may recommend and prescribe chemotherapy, radiation, or even newer immunotherapy options. --- ## Propecia (finasteride) **Link:** [Propecia (finasteride)](https://www.oceandrivedermatology.com/skin-conditions/propecia/) ([md](https://www.oceandrivedermatology.com/skin-conditions/propecia/index.html.md)) **Type:** Service **Description:** Propecia is the brand name for finasteride 1 mg tablets. Finasteride tablets have been around for a long time for the treatment of benign prostate hyperplasia ### Content **Propecia** is the brand name for finasteride 1 mg tablets. **Finasteride** tablets have been around for a long time for the treatment of benign prostate hyperplasia (BPH) in men in the 5 mg dosage. During BPH treatment some men started to notice that they were growing hair on their scalp or that their thinning hair appeared thicker. Subsequently, research went into finasteride as a possible **treatment for male pattern hair loss**. During the testing phase of finasteride it was determined that 1 mg was sufficient to help halt hair loss and promote hair growth. Propecia 1 mg tablets are taken daily and it will take a minimum of 6 to 12 months to determine its efficacy. I tell my hair loss patients that the primary goal of finasteride is to *halt or slow the hair loss progression*. If we can reverse some of the hair loss that is even better. Patients often ask me if Propecia is the only thing they need to do for their hair loss. Unfortunately, hair loss is best treated by multiple modalities. Typically, I will combine multiple therapies to get the best results. **Women can also take finasteride for female pattern hair loss**, typically at 5 mg daily, but must be post-menopausal or cannot have children due to the teratogenicity of the medication. Alternatively, a medication such as spironolactone may be prescribed alongside a birth control pill to those patients that are premenopausal. As is for men, it will take 6 to 12 months to determine its efficacy. With Propecia or finasteride treatment the results only last as long as the medication is taken. Pattern hair loss will begin to take over again in about 6 months after stopping treatment. --- ## Psoriasis **Link:** [Psoriasis](https://www.oceandrivedermatology.com/skin-conditions/psoriasis/) ([md](https://www.oceandrivedermatology.com/skin-conditions/psoriasis/index.html.md)) **Type:** Service **Description:** Commonly you will see these thick plaques on peoples elbows and knees, but can also appear other areas of the body. These plaques can be small and dime sized or large and silver dollar sized. ### Content There are a few types of **psoriasis** with the most common and recognizable being plaque psoriasis. This can affect the skin from the scalp all the way to the feet. Commonly you will see these thick plaques on peoples elbows and knees, but can also appear other areas of the body. These plaques can be small and dime sized or large and silver dollar sized. Just like eczema and allergic contact dermatitis, **psoriasis can be itchy**. Another form of psoriasis that can occur is called guttate. This is just a fancy word for drop or tear drop shaped plaques. Many times, **guttate psoriasis** appears after catching a cold; specifically, a cold caused by streptococcus infection. If you had a cold with sore throat a few weeks ago and all of a sudden develop a rash that looks like psoriasis you might have guttate psoriasis. Unlike plaque psoriasis that tends to have large thick lesions these tend to be much smaller and thinner. The good news is that guttate psoriasis usually resolves and is a one-time problem. A small number of patients may develop life-long psoriasis after the development of guttate psoriasis. **Pustular psoriasis** is a rare form of psoriasis and can potentially be life-threatening. This form of psoriasis can happen while pregnant, secondary to some type of infection, or even after being treated with oral steroids for psoriasis. This is why we tend to stay away from oral steroids when treating psoriasis. **Inverse psoriasis** is just a way to describe psoriatic areas on the body that are in the skin folds. The groin, arm pits, perianal, under the breasts could all be locations where inverse psoriasis develops. Another common form is **nail psoriasis**. This can develop by itself, but typically is in conjunction with psoriasis of the skin. We tend to see more psoriatic arthritis in patients with nail disease. Nail psoriasis is sometimes difficult to treat because the problem is coming from where the nail grows (the nail bed), not necessarily the nail itself. You may notice color changes in your nail, texture changes, or pitting of the nail. We are starting to identify other systemic conditions associated with psoriasis. For instance, it is well documented that uncontrolled diabetes can lead to cardiovascular complications such as stroke. We are now learning that psoriasis can be associated with cardiovascular disease, high blood pressure, and even diabetes. Although there is no agreed upon reason these diseases are associated with psoriasis; I tend to believe it is the **chronic inflammation of psoriasis**. Luckily, there are many **treatment options for psoriasis**. Just turn on the TV and you will see commercials for psoriasis or psoriatic arthritis all the time. This is good news for patients. There is a lot of research going into treatments so every year we tend to see new treatment options. But, just because they are new does not necessarily mean they are better. They just provide another treatment option, which is always good a thing. Typical psoriasis prescriptions can include: topical steroid ointments and creams, medications that help thin out the thick plaques, oral medications like apremilast (Otezla), methotrexate, acitretin (Soriatane), and injectable medications such as adalimumab (Humira), ustekinumab (Stelara), and ixekizumab (Taltz). Sometimes a combination of treatments is needed to obtain good clearance. Sometimes a single medication does the trick. Remember that we do not yet have a cure for psoriasis, but we have plenty of good treatment options to help. #### What is Psoriasis? Psoriasis is a chronic or long-term inflammatory condition that affects the skin and nails. It can also have an internal component called psoriatic arthritis. Psoriatic arthritis is typically treated along with a rheumatologist and psoriasis of the skin by a dermatologist. Most people can identify psoriasis by its classic red, thick, scaly plaques on the body. And those with a family history of psoriasis may be more likely to develop this skin condition than someone without a family history. Interestingly, there are several other risk factors for developing psoriasis. Some may make sense and others may surprise you. Smoking, obesity, certain medications like beta blockers or lithium, and alcohol have all been studied and shown to be correlated with psoriasis. This does not mean that every person that takes a beta blocker for their heart condition will develop psoriasis, but it is something to be aware of. There are a few types of psoriasis with the most common and recognizable being plaque psoriasis. This can affect the skin from the scalp all the way to the feet. Commonly you will see these thick plaques on peoples elbows and knees, but can also appear other areas of the body. These plaques can be small and dime sized or large and silver dollar sized. Just like eczema and allergic contact dermatitis, psoriasis can be itchy. --- ## Ablative vs Non-Ablative Lasers **Link:** [Ablative vs Non-Ablative Lasers](https://www.oceandrivedermatology.com/laser-treatments/ablative-vs-non-ablative-lasers/) ([md](https://www.oceandrivedermatology.com/laser-treatments/ablative-vs-non-ablative-lasers/index.html.md)) **Type:** Service **Description:** Ablative lasers are excellent at targeting collagen and elastin fibers that live in the dermal layer of the skin. Stimulating collagen and elastin fibers are one of the most important aspects of laser therapy. ### Content #### Ablative Lasers Laser therapy tends to be a topic that gets confusing, quickly. Just Google CO2 laser, erbium laser, [fractional laser](/laser-treatments/profractional-laser/), IPL, [BBL Laser](/laser-treatments/bbl-hero/), nano peel and you will see what we are talking about. One of the easiest ways to differentiate lasers is between ablative and non-ablative. **Ablative lasers** can remove skin from just the epidermal (first) layer or both the epidermal (first) layer and dermal (second) layers of the skin. Ablative lasers can remove 100 percent of the skin or just a percentage or fraction of the skin. **Fractional lasers** are ablative lasers. However, fractional laser does not remove 100 percent of the skin during treatment. Fractional lasers are essentially creating microscopic channels in the skin, not dissimilar to aerating your lawn. One of the first-generation ablative lasers was the carbon dioxide laser or CO2 laser. This laser was able to address numerous skin issues in a single treatment such as: skin tone, texture, pores, and removing fine lines and wrinkles. However, CO2 ablative laser sometimes came with a lot of downtime, extended redness, and a higher incidence of adverse effects such as permanent hypopigmentation of the skin. This tended to be common in the perioral area. Today, the erbium ablative laser is very common and what we utilize in our offices. **Erbium can provide similar results to CO2, reduce the amount of side effects, and reduce downtime.** #### Why are ablative lasers popular? **Ablative lasers are excellent at targeting collagen and elastin fibers** that live in the dermal layer of the skin. Stimulating collagen and elastin fibers are one of the most important aspects of laser therapy. *Collagen and elastin are what help plump up the skin* and allow it to return to its original position after being stretched. Some ablative lasers can stimulate collage for 9 to 12 months post-procedure. **Ablative lasers can also help with pore size**, tone and texture of the skin, and give the skin an overall brilliant appearance. #### What is the downtime like for ablative lasers? Downtime varies from a few days to a few weeks depending on the actual treatment being completed. A “nano peel” that only targets the first layer of skin may cause a sunburn like appearance that heals in just a few days. A **fractional laser treatment** may take 5 to 10 days and a 100 percent full ablative procedure maybe take up to three weeks to heal. And a **scar treatment with fractional laser** may only take a couple days to heal. #### What are some side effects of ablative lasers? Side effects can occur with ablative lasers. This may depend on the depth of treatment, the aggressiveness of the treatment, and even the skin type of patient. Potential side effects can include, but are not limited to pain, reddening of the skin, swelling, pigment changes (including increased pigmentation – skin darkening/reddening and decreased pigment – skin lightening or whitening). **Pigment changes lasting 1-6 months or longer are rare but may occur.** #### Non-Ablative Lasers **Non-ablative lasers** use pulses of light to heat up the skin, but do not actually remove any tissue or skin during the treatment. Downtime is easier and recovery is quicker compared to ablative lasers. --- ## BBL HERO **Link:** [BBL HERO](https://www.oceandrivedermatology.com/laser-treatments/bbl-hero/) ([md](https://www.oceandrivedermatology.com/laser-treatments/bbl-hero/index.html.md)) **Type:** Service **Description:** Experience BBL HERO at Ocean Drive Dermatology in Vero Beach, FL. The **BBL HERO** is a fast, permanent, all-over laser treatment that effectively reduces the visibility of hyperpigmentation caused by sun damage, age spots, redness, and other factors, while also reducing wrinkles, fine lines, and dull skin. This advanced technology emits strongly pulsed light (IPL wavelengths) faster than previous BBL treatments, making it *one of the most effective BBL treatments currently available*. **Rediscover beautiful skin at Ocean Drive Dermatology in Vero Beach, FL.** #### Treats Larger Areas *What is BBL Laser?* At Ocean Drive Dermatology in Vero Beach **BBL Laser** treatment sets a new standard in treating the skin to remove years of sun damage or help control redness or erythema caused by rosacea. And it is a fast treatment. BBL HERO delivers the treatment up to 4x faster than before, provides up to 3x the peak power, and 2x the cooling than the original BBL. This allows us to treat larger areas, such as the back, arms, and legs, in as little as five minutes (depending on the size of the area treated). In a nutshell, BBL HERO is the BBL you already love, but better! #### Natural CellRenewal & Repair *What are the benefits of the BBL HERO?* If you want to improve the look of your skin but don't want to undergo any invasive surgical procedures, **BBL HERO could be the ideal treatment for you**. Intense pulsed light (BBL) is an effective treatment for a variety of skin conditions, including rosacea, acne, and sun damage. When it comes to eliminating acne, **BBL HERO** is unrivaled. Because it speeds up the skin's natural process of cell renewal and repair, the treatment can significantly reduce the signs and symptoms of this widespread condition. BBL is most commonly used to treat sun damage and hyperpigmentation on the face, but it is also beneficial on the arms, chest, hands, legs, and neck. #### Head-to-Toe —We Have You Covered *Your BBL HERO Consultation* At Ocean Drive Dermatology, we understand that our patients lead busy lives. That's why we're proud to offer the BBL HERO, which can treat the entire body in just 30 minutes. This makes **BBL Laser treatments** accessible to everyone, even the busiest of people. **Come visit us at Ocean Drive Dermatology in Vero Beach, FL, to experience the benefits of BBL HERO and achieve radiant, glowing skin.** #### Optimal Results *BBL Laser Results* When treating vascular issues, especially on the face, know it will take multiple treatments to get the best results. Sometimes vessels look purple and then fade away; other times, they look more prominent and then, over a two-week period, as less noticeable. And if you are having your rosacea treated, it is important also to be seen for an insurance (or very reasonable self-pay options) visit in our dermatology office. The best results come when you combine BBL with prescription treatments. Because **BBL is a no-downtime procedure** (for most patients), it is ok to apply makeup over the top of any blotchiness or dark brown spots during recovery. #### Minimal Downtimeto Reset Your Skin *BBL HERO Recovery and Aftercare* It is not uncommon to have blotchy appearing skin for 12 to 36 hours post-treatment, along with a sensation of a mild sunburn for the same length of time. *Gentle skincare products are recommended during recovery*, including being vigilant about sun protection for about one month post-treatment. Brown spots tend to look darker (almost the color of coffee grounds) and take about ten to 14 days to disappear on the face. Anywhere else on the body tends to take longer, up to 21 days, to disappear. Sometimes we combine BBL with topical prescription medications to further improve brown spots that you can use between treatments or to help maintain your results. These medications may contain hydroquinone, tretinoin, fluocinolone, kojic acid, and vitamin C, along with a good daily sunscreen. #### BBL Hero *Frequently Asked Questions* --- ## Seborrheic Dermatitis **Link:** [Seborrheic Dermatitis](https://www.oceandrivedermatology.com/skin-conditions/seborrhea-seborrheic-dermatitis/) ([md](https://www.oceandrivedermatology.com/skin-conditions/seborrhea-seborrheic-dermatitis/index.html.md)) **Type:** Service **Description:** Seborrheic Dermatitis is a skin condition that typically affects the scalp, face, and even chest, arms pits, and groin areas on the body. ### Content #### Where did these greasy and itchy spots in my scalp come from? **Seborrhea** is a skin condition that typically affects the scalp, face, and even chest, arms pits, and groin areas on the body. Anyone can get this skin disease. Typically, patient’s will tell me their scalp itches and they notice red greasy scales that tend to flake off if they pick at the area. These red greasy scales can also occur in front of and behind the ears, the nasolabial folds (by the nose), and even in the eyebrow areas on the face. We don’t know exactly the cause of seborrhea but believe a form of yeast called Malassezia furfur or M. furfur for short might be the culprit. This yeast is common to everyone’s skin and not contagious. #### Treatment Options The good news is there are **many treatment options available for Seborrheic Dermatitis**. Treatment tends to calm down the symptoms and for some patients completely resolve the symptoms. Because this M. furfur yeast lives on our skin all the time it is possible for the seborrhea to come back. It’s not uncommon for this disease to wax and wane. **Seborrheic Dermatitis treatments** typically include antifungal shampoos, creams and topical steroids. Remember that seborrhea is not contagious and can usually be controlled with proper medications. --- ## Rosacea **Link:** [Rosacea](https://www.oceandrivedermatology.com/skin-conditions/rosacea/) ([md](https://www.oceandrivedermatology.com/skin-conditions/rosacea/index.html.md)) **Type:** Service **Description:** Rosacea tends to occur in fair skin individuals after age 30 and is fairly common. Most people with rosacea notice skin color changes such as red cheeks or even “pimples” popping up as if they were a teenager again. ### Content #### What are these red pimples on my face? I’m not a teenager anymore. **Rosacea** tends to occur in fair skin individuals after age 30 and is fairly common. Most people with rosacea notice skin color changes such as red cheeks or even “pimples” popping up as if they were a teenager again. We aren’t really sure why people get rosacea. A few possibilities include inflammation to certain organisms on the skin, sun damage, and vascular malfunction as we age. **Rosacea** presents on the skin in different ways. Some patients will notice long-term redness of the nose and cheeks. Others will notice red pimple-like bumps on their face called papules and pustules. Many complain of flushing of their cheeks along with broken blood vessels around their nose, cheeks, and chin. About 50 percent of patients note involvement in their eyes. Long term rosacea can even include skin texture changes and thickening of the nose and chin. Unfortunately, many patients with rosacea suffer from multiple symptoms; not just one. #### What makes rosacea worse? We know that certain things can make rosacea worse. Sun exposure, hot beverages, spicy foods, alcohol and even exercise can flare up rosacea. This does not mean you shouldn’t go out in the sun or eat spicy foods, but it does mean you might want to limit it or take appropriate precautions. **Daily sunscreen is a must for those with rosacea**. SPF 30 with zinc oxide or titanium dioxide is our recommendation. If you are going out to dinner on a first date maybe avoid spicy food so you don’t get flushed in the cheeks. #### Treatment Options The good news is there are plenty of **treatment options for rosacea**. Everything from topical medications (Metronidazole, Soolantra, Plexion, Mirvaso), oral medications (Minocycline, Doxycycline, Oracea), and even light therapy such as [BBL (broadband light)](/laser-treatments/broadband-light-bbl/) can all **reduce your rosacea symptoms**. The most important thing to remember about rosacea is that it is a long-term disease process. Although we can almost always control the disease process we don’t yet have a cure. A lot of times we can reduce treatments once the disease is under control, but almost always we need to continue some type of treatment to keep the disease at rest. Just as important as the treatment is prevention. We know that wearing sunscreen daily and avoiding other triggers such as spicy food, hot beverages, and alcohol can really help. --- ## Telogen Effluvium Treatment **Link:** [Telogen Effluvium Treatment](https://www.oceandrivedermatology.com/skin-conditions/telogen-effluvium/) ([md](https://www.oceandrivedermatology.com/skin-conditions/telogen-effluvium/index.html.md)) **Type:** Service **Description:** Telogen Effluvium is a process that causes excess hair shedding to occur and is noticeable in brushes, pillows, and around the shower drain. ### Content This process causes excess hair shedding to occur and is noticeable in brushes, pillows, and around the shower drain. The good news is this hair shedding process will not cause you to go bald. **Telogen Effluvium** is typically a temporary shedding process that will reset within 12 months. During this time you will most likely notice shorter hairs growing along the temple areas and even along the center part line in the scalp. At Ocean Drive Dermatology in Vero Beach *telogen effluvium is usually associated with a stressful event* several months prior to the actual shedding process. Events such as extreme stress, loss of a loved one, starting or stopping a long term medication, surgery, anesthesia, illness etc. Usually we are able to identify an event that may have caused this process to start. Although rare, it is possible to have chronic telogen effluvium where you simply shed more than you did in the past. There are some underlying disease processes that can be checked by obtaining blood work, but typically these results are normal. Even if there is an underlying bloodwork abnormality, it is not a guarantee that was the cause of telogen effluvium. Supplements such as Biotin, Viviscal, and Biosil are all options that can be considered when **treating telogen effluvium**. These supplements support the hair growth cycle and help resist further shedding. --- ## Squamous Cell Carcinoma **Link:** [Squamous Cell Carcinoma](https://www.oceandrivedermatology.com/skin-conditions/squamous-cell-carcinoma/) ([md](https://www.oceandrivedermatology.com/skin-conditions/squamous-cell-carcinoma/index.html.md)) **Type:** Service **Description:** Squamous cell carcinoma (SCC) is a very common type of skin cancer that can occur anywhere on the body that has had too much sun. ### Content #### Squamous Cell Carcinoma (SCC) Early-stage skin cancer is highly curable. The team at Ocean Drive Dermatology & Plastic Surgery, located in Indian River Shores and Vero Beach, FL, diagnoses skin cancer early with annually cancer screenings, available to patients throughout the greater Treasure Coast area. **Squamous cell carcinoma** (SCC) is a very common type of skin cancer that can occur anywhere on the body that has had too much sun. In fact, it is the second most common with about one million diagnosed each year. Just like other skin cancers, those with fair skin and long-term sun exposure are at greater of developing one. Also, not surprisingly, **we tend to see SCC in older patients** as it takes time for the cumulative effects of the ultraviolet exposure to take its toll. With that said, it’s not unheard of for someone under 40 to be diagnosed with a SCC, especially in southern Florida and those with a history of using a tanning bed. The most common location to develop a SCC is on the head or neck. Although we tend to refer to a squamous cell skin cancer as a **squamous cell skin cancer** there are actually some sub-types of SCC. Squamous cell carcinoma in-situ, SCC-IS, Bowen’s Disease. These are all the same thing. Basically, SCC-IS means that the skin cancer only involves the outermost layer of skin called the epidermis and has not gone into the second layer of skin called the dermis. Invasive SCC. When people hear the word invasive they tend to think it means the SCC had spread to other parts of the body. This is not typically the case. In fact, it is rare that a **squamous cell skin cancer** would spread beyond the skin. We think the number is between two and five percent of SCC spread. Invasive SCC goes past the epidermis and into the dermal layer of skin; sometimes even to deeper structures of the skin. Keratoacanthoma or KA. Although this does not have squamous cell in the name we tend to lump this in the same category because it resembles and acts like SCC. In fact, when looking at a KA under the microscope it’s not always easy to tell the difference between it and an SCC, thus why we typically put it in the same category as SCC. There are several **treatment options for SCC** based on location of the skin cancer (face vs back) and type (SCC-IS vs invasive). Treatment could include [surgical excision](/skin-cancer/excisions/), [Mohs micrographic surgery](/skin-cancer/slow-mohs/), electrosurgery (ED&C, [electrodesiccation and curettage](/skin-cancer/electrodessication-curettage-edc/)), radiation therapy, and topical therapies such as fluorouracil (Efudex). **The prognosis for SCC is very good**. Once the skin cancer is removed or otherwise treated it is extremely important to have good follow up. Like all cancer, skin cancer can recur in the same treated area or develop in a new area. We recommend a total body skin exam every six months for two years after the diagnosis of a squamous cell skin cancer. If no other skin cancers are found during that time period we then recommend yearly total body skin examinations. Possibly more important is the need to look over your body once a month at home. Trust your instincts. If a spot does not heal, bleeds, or somehow is different from your other moles you should come in for an office visit sooner. --- ## CO2 Laser **Link:** [CO2 Laser](https://www.oceandrivedermatology.com/laser-treatments/co2-laser/) ([md](https://www.oceandrivedermatology.com/laser-treatments/co2-laser/index.html.md)) **Type:** Service **Description:** With the CO2 laser treatment in Vero Beach, you can get the glowing, healthy skin you’ve always wanted. At [Ocean Drive Dermatology](/) in Vero Beach, Fl, we utilize two types of ablative lasers in our offices. The first is erbium, which is part of our Sciton family of lasers (Contour, ProFractional, Halo, BBL, BBL HERO, ClearV, ClearSilk, and MicroLaser Peel). The second is CO2, part of our Rohrer Aesthetics family of lasers and energy devices (Phoenix, Pixel RF Microneedling, Bodytone, and Bodysculpt). **Both ablative lasers excel at resurfacing**, scar revision, and bringing back that healthy glow to your skin. #### What can CO2 Laser Treatment do for me? In Vero Beach **CO2 lasers** are commonly used on the face, neck, chest, arms, and hands to help *remove brown spots, stimulate collagen and elastin proteins, tighten pores, and reduce lines and wrinkles*. Originally, CO2 lasers were developed for deep resurfacing and sometimes caused pigment changes, specifically hypopigmentation. The Phoenix is not prone to this type of problem because it utilizes a Micro-Thermal Zone Scanner, which divides the laser beam into small shafts of light. We call this a **CO2 fractional laser**, which is *safer than full-field resurfacing* while still providing many of the same benefits as full-field resurfacing. Depending on the area we are treating, the laser can be fine-tuned to treat an area as small as 5 mm, all the way up to 30 mm in a single pass. #### Results, Recovery and Aftercare Following a treatment with the **CO2 laser** Vero Beach patients will have downtimes similar to other fractional lasers that we operate daily. Their downtime could be as little as a few days all the way up to several days until healed. The downtime is related to how aggressive the treatment is, but the results are also related to how aggressive the treatment is. More downtime typically equals more results. And just like our other lasers, we offer *CO2 resurfacing* with our surgical procedures in the operating room with Dr. Durkin and Dr. Chim. This allows you to have one set of downtime from both your surgical and laser procedure. However, the majority of CO2 resurfacing we do is awake in our Laser Center at Ocean Drive Dermatology. #### Is CO2 Laser Treatment Painful? Just like all the other lasers at Ocean Drive, we offer specialized topical numbing medication and ProNox to make the treatment more comfortable while enabling you to drive to and safely from the office. Depending on the area treated, we offer medical-grade recovery products, as well as red light therapy the day after treatment to speed up the healing process. **CO2 fractional laser treatment can be combined with other lasers** at Ocean Drive Dermatology in Vero Beach, Florida. #### Schedule Your Vero BeachCO2 Laser Consultation *at Ocean Drive Dermatology Today* Do not hesitate to contact Ocean Drive Plastic Surgery Dermatology and Medspa, located in beautiful Vero Beach, FL, for a **complimentary CO2 laser consultation**. This is a rash that typically occurs during the warmer months of the year and can appear as *white, salmon colored, or even light brown patches* on the chest, arms, and legs. You may also notice some flakes or scales with the rash, but usually no itching symptoms. And because we live in Florida, I tend to see this rash all year long. I also notice this occurs more in my patients that like to exercise or participate in sports as they tend to sweat more than others. Why versicolor in the name? This rash can present in several different colors as noted above, such as white, pink, and brown. Just like seborrheic dermatitis, we think this disease process called **tinea versicolor** is caused by a yeast called Malassezia furfur, among others such as Malassezia globose. #### Tinea Versicolor Treatment Options **Tinea Versicolor treatment** typically includes shampoos, use of a body wash, creams, and sometimes even pills to help reduce the amount of yeast on the skin. Some lifestyle modifications include showering immediately after exercising and not wearing tight fitting clothing. Remember, that once you get tinea versicolor you can always get it again. Treatment may be one and done or treatment may be ongoing sporadically to control the symptoms. --- ## Skin Cancer **Link:** [Skin Cancer](https://www.oceandrivedermatology.com/skin-cancer/) ([md](https://www.oceandrivedermatology.com/skin-cancer/index.html.md)) **Type:** Service **Description:** At Ocean Drive Dermatology in Vero Beach, we provide services and procedures related to Skin Cancer and Skin Cancer treatment. It’s not uncommon to have multiple AKs at one time. Both men and women with fair skin and those with multiple sun burns throughout their lives are more at risk of developing an AK. These premalignant cells typically cause a red rough spot to appear on the skin. A lot of times it feels like sand paper when you touch it and sometimes that is the only way you know it’s there. They don’t always have color; only texture. It’s not uncommon to find these spots on your scalp, forehead, nose, lips, cheeks, ears, chest, forearms, and tops of hands. As you can see these can pop up anywhere on your body where there has been too much sun exposure. Because **actinic keratosis is a premalignant skin condition** it does have the potential to turn into a common form of skin cancer; specifically, [squamous cell carcinoma](/skin-conditions/squamous-cell-carcinoma/) (SCC). The good news is not all actinic keratoses turn into squamous cell skin cancer, but some do. This is the reason it is important to treat these spots before they transform into a skin cancer. #### Treatment Options **Treatment for actinic keratosis** is fairly straight forward. There are treatments that you can do at home and some require a trip to the office. Some at home treatments include fluorouracil (Efudex), imiquimod (Aldara), and ingenol mebutate (Picato). These creams are applied anywhere from days to weeks and typically reserved for patients who have numerous AKs rather than just a few. Side effects are common, such as red irritated skin that can turn into scabs. Afterwards, once your skin has a chance to recover from treatment, typically you are left with *much smoother skin* and most importantly, less AKs. Unfortunately, this is not a guarantee you’ll never have another AK again. An in-office treatment can include liquid nitrogen cryotherapy. This is the most popular treatment for AKs. It is quick, the cure rate is high, and most patients tolerate it just fine. The treated area turns red, sometimes blisters, and in about 14 days resolves. Another treatment you need to come into the office for is [photodynamic therapy, PDT](/skin-cancer/photodynamic-therapy-pdt/) or “Blue Light”. This is a multi-step process where a photosensitizing medication is applied to the treatment area and then activated by the blue light. This is typically used for the face, scalp, and ears. Expect a sunburn afterwards and healing times anywhere from one to a couple weeks. #### Combine Treatment Options Sometimes multiple therapies are combined to give the best outcomes. For instance, you may be asked to use Efudex for 5 days and then immediately be treated with PDT. Or maybe some thicker AKs will be thinned out with a curette before PDT. Everyone is different and each treatment is tailored to the specific patient to obtain the best possible outcome. Remember that it is important to continue to follow up with your skin care provider on a regular basis even after treatment. --- ## Atopic Dermatitis **Link:** [Atopic Dermatitis](https://www.oceandrivedermatology.com/skin-conditions/atopic-dermatitis/) ([md](https://www.oceandrivedermatology.com/skin-conditions/atopic-dermatitis/index.html.md)) **Type:** Service **Description:** Dermatitis just refers to inflammation of the skin. Sometimes dry skin turns into very red and very itchy skin. ### Content #### Eczema and Atopic Dermatitis **Dermatitis** just refers to inflammation of the skin. Sometimes dry skin turns into very red and very itchy skin. This might be a clue that you have eczema or atopic dermatitis, which is an inflammatory condition affecting the skin. Sometimes there is an underlying culprit such as medications or taking too hot/too long showers, but a lot of times there is a dysfunction in the epidermal (top layer of skin) moisturize barrier of your skin. This dysfunction in the skin can occur at a very young age and be a life-long condition. Some patients also have a history of asthma and seasonal/year-round allergies, which can clue us in on the diagnosis. Clues that you might have more than dry skin can include **redness or even oozing and crusting of the skin**. If you notice that no matter how much you moisturize and how cool a shower you take your skin is still itchy, red, and even oozing it is time to make an appointment. Left untreated, it is possible to develop a skin infection. There are **atopic dermatitis treatment** options that can really make a difference. Along with moisturizers, luke-warm showers, we can add prescription medications to the treatment plan. Topical steroid creams such as triamcinolone, or non-steroid creams such as pimecrolimus (Elidel) and Crisaborole (Eucrisa) to oral medications such as prednisone and even injectable medications like dupilumab (Dupixent) are all potential treatment options. In rare cases nonsteroidal immunosuppressive medications such as methotrexate or mycophenolate could be utilized. Depending on the severity of your **dermatitis** we may combine treatments to get the best outcomes and even add on an antibiotic if there is a skin infection. If itching is a main concern of the patient additional medications may be offered. Sometimes a simple over the counter or OTC antihistamine such as fexofenadine (Allegra) may be recommended. If that’s not doing the trick prescription medications such as hydroxyzine (Atarax) may be an option. There are also topical medications that can help with itch. A great over the counter option is Sarna Sensitive Lotion that has the ingredient with pramoxine. If you see the store brand next to it, that is ok to purchase as long as it has the same active ingredients. Pramoxine is powerful anti-itch medication that can also help moisturize the skin. With an arsenal of treatment options, our Vero Beach **atopic dermatitis treatment** providers can find something to help control your red and itchy skin. Sometimes several medications need to be tried before finding the right combination of treatments that help heal your skin. Remember that eczema or atopic dermatitis tends to be a life-long battle. Over time we can usually decrease the amount of prescription medications that are needed. --- ## Juvederm **Link:** [Juvederm](https://www.oceandrivedermatology.com/injectables-fillers/juvederm/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/juvederm/index.html.md)) **Type:** Service **Description:** Smooth lines, sculpt contours, and restore volume with the Juvéderm Collection. Warts are caused by the human papilloma virus (HPV) ### Content #### Where do warts come from? Almost everyone knows someone that has had a wart or has had a wart themselves. **Warts (or verruca) are caused by the human papilloma virus (HPV) and are very common**. Children to adults can get warts. Sometimes they are rough and raised and sometimes flat and smooth. Almost always painless. Patients that are immunosuppressed from a transplant or HIV are at risk for warts. HPV is typically harmless and there are literally hundreds of subtypes of this virus. The good news is most common warts such as subtype 2, 3, 4, 27, etc. are harmless. These are different subtypes than the vaccine that adolescents get to protect themselves from cancerous sexually transmitted strains of HPV. That is why it is possible to still get a wart on the bottom of your foot (plantar wart) even if you had the vaccine. Warts can occur anywhere on the body from the head down to the toes and everywhere in between. **Treatments for warts** are available over the counter, but if those have not helped, I recommend scheduling an appointment to see us. If you are interested in learning more about **wart removal** Vero Beach patients are encouraged to contact us to schedule a consultation. #### Wart Removal Options We offer [cryotherapy](/skin-cancer/cryotherapy/), which uses liquid nitrogen to help destroy the wart. This is similar to Dr. Scholl’s “freeze away”, but much stronger. Other treatment options are prescription topicals that are applied nightly, as well as in-office immunotherapy injections with candida or trichophyton. I tell all my Vero Beach **wart removal** patients that they are sometimes tough to get rid of. Your body does not seem to care that they are present and even with treatment it is not uncommon to have multiple treatments with cryotherapy before the wart resolves. So hang in there, eventually the wart will go away. If you happen to find a wart in your genital area, then I always recommend completing STD/STI testing. There are even certain types of warts that affect the genital area that are cancerous called Bowenoid papulosis. Typically, this is diagnosed with a biopsy of the suspected wart. And yes, you should refrain from sexual intercourse if you think you have a wart in the genital area. #### Don’t pick! Whatever you do, don’t pick or scratch at the wart. They can be spread by self-inoculation. This means you can transfer the wart from one area on your body to another. --- ## Radiesse **Link:** [Radiesse](https://www.oceandrivedermatology.com/injectables-fillers/radiesse/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/radiesse/index.html.md)) **Type:** Service **Description:** Restore volume, lift, and smooth wrinkles with Radiesse. At Ocean Drive Medspa in Vero Beach, Restylane offers a unique blend between lifting tissue, and filling tissue. ### Content **Restylane** is the original hyaluronic acid filler, and has the distinction of being *the most injected dermal filler in history*. Since the beginning of dermal filler therapy, more syringes of it have been injected than any other commercially available dermal filler. Restylane-L is the original Restylane, but it has 1% lidocaine numbing solution added to the filler. This provides better pain control during the injection, and as such Restylane-L has essentially replaced Restylane in the United States Market. **Restylane** again offers a unique treatment profile that distinguishes it from the other dermal fillers. It is known to elegantly lift tissues after injection, as it has a very [high g’prime](https://www.oceandriveplasticsurgery.com/injectables/face-fillers/). Remember, the degree of cohesivity (aka g’prime) that a filler has defines its ability to lift tissue after injection. It also has the second highest g’prime, after Radiesse. Unlike Radiesse, however, it is a hyaluronic acid filler, and as such, does have some degree of hydrophilicity, also known as water absorption. Translated, this means that it offers a unique blend between lifting tissue, and filling tissue. While most fillers will either lift or fill, **Restylane offers a unique blend of both tissue lift and fill**. Results from it are seen instantaneously, but mature over the course of 48 hours as water is added into the gel from the surrounding tissue. **Restylane is a reversible filler**, and can be removed through the injection of an enzyme called hyaluronidase. While reversal is exceedingly uncommon, many of our patients like to know that their results can be “tweaked” to perfection following injection. Restylane-L is approved for use throughout the face, but we find it to be most effective in the following scenarios: - Erasing or minimizing the line between the lower eyelid and cheek, known as the “tear trough”. Because of its unique combination of lift and limited water absorption, **Restylane** is the best available dermal filler for treatment of this complex area. For these injections, we commonly will further mix extra 1% lidocaine into the gel prior to injection to minimize any discomfort during the procedure. Results are instantaneous, and typically last nine to fifteen months. - Dermal filler for treating mild to moderate nasolabial fold hollowing in patients without significant cheek droop. Direct injection of Restylane-L into patients with mild to moderate hollowing produces natural, elegant results in an instantaneous fashion. Results typically last nine to fifteen months. - Nonsurgical evening out of rough bones and cartilage in the nose. For people who have indentions and bumps on the roof of their nose, they can be easily camouflaged with Restylane-L injections. While these injections do not directly address the root problem, it can mask their appearance without the need for invasive surgery. This is a very popular treatment in men, and in patients who have previously undergone rhinoplasty. - Commonly used around the eyes to rejuvenate the brow area. Injection of Restylane-L into the lateral brows can have a soft, lifting effect on the brow position, and can brighten and soften almost anyone’s look!! Restylane-L is an incredibly useful filler, and can be adapted for use in almost any area of the face. However, it does not offer as much lift to the cheek area as other fillers, and as such, we limit usage of it in the malar/cheek area. Beyond that, Restylane can be used in almost any other area safely, with an excellent result profile. Our Vero Beach **Restylane** patients will typically receive between 1-4ml per treatment. --- ## Ablative Laser Resurfacing **Link:** [Ablative Laser Resurfacing](https://www.oceandrivedermatology.com/laser-treatments/ablative-laser/) ([md](https://www.oceandrivedermatology.com/laser-treatments/ablative-laser/index.html.md)) **Type:** Service **Description:** Get noticed with ablative laser treatments at Ocean Drive Dermatology in Vero Beach! At Ocean Drive Dermatology in Vero Beach **ablative laser resurfacing** can keep them guessing about your age. Located in Vero Beach, Florida, our medical providers will develop a unique treatment plan to leave you looking and feeling your best. **Schedule your ablative laser treatment consultation today.** #### What is theContour TRL (tunable resurfacing laser)? *What is an Ablative Laser?* The Contour TRL by Sciton is a full-field ablative erbium: YAG laser that is fully customizable. A full-field **ablative laser** is different from our [ProFractional Laser](https://www.oceandrivedermatology.com/laser-treatments/profractional-laser/). When using the Contour TRL, we are removing 100 percent of the targeted skin layer in the treatment area, while a fractionated laser is simply creating microscopic holes in the skin, thus only treating a “fraction” of the treatment area. Depending on the depth, we can *remove just a thin layer of the epidermis* (top layer of skin), and we call this a MicroLaser Peel. Alternatively, we could choose to really target rhytides or “wrinkles” and completely remove the first layer of skin and then start meticulously removing the second layer of skin called the dermis. This more aggressive treatment can produce some **amazing results by improving wrinkles, texture, and tone of the skin**. Below we will discuss more about this laser and who would benefit from it. #### Keep them guessing. *What are the benefits of an ablative laser?* Get ready for questions. “Did you get work done?” “When did you find the time to recover?” Ablative laser treatments are minimally invasive with minimal downtime, which is why this is such a popular cosmetic skin rejuvenation treatment. **Ablative laser treatments offer many benefits**, such as: - Smoothing skin and reducing fine lines and wrinkles. - Treating and minimizing acne and injury scars. - Reducing age spots, sun damage, and other hyperpigmentation - Brightening and rejuvenating your complexion. - Stimulating collagen production and firming skin #### Experiencethe best skin of your life. *Your Ablative Laser Consultation* Book a Vero Beach **ablative laser resurfacing** consultation at Ocean Drive Dermatology to learn more about this amazing procedure. It is possible to achieve a radiant complexion, improved skin tone and texture, and a newfound confidence that's sure to turn heads. **Don't wait to look and feel your best. Schedule your consultation today.** #### Long-lasting Improvement *Ablative Laser Results* **Recovery after ablative laser resurfacing** may take a few days to a few weeks for complete healing, depending on the intensity of your treatment. There are [additional laser add-on treatments](https://www.oceandrivedermatology.com/laser-treatments/) to help speed up the healing process. Your skin's quality and look will improve once the treated area heals. Protect your skin from the sun at all times after laser resurfacing. New sun damage can undermine your efforts. It is best to use a moisturizer and sunscreen with an SPF of 30 daily. #### You’re worth it. *Ablative Laser Recovery and Aftercare* The first few days after treatment, we see you in the office every day for Oxy-Mist treatments to help speed your recovery and confirm everything is healing appropriately. We will provide you with all the products needed to assist in your skin recovering as fast as possible. After the first week, you can usually change over to a hydrating moisturizer and cleanser. Usually, by ten days to two weeks, the skin has healed. However, you may notice that your skin is still red or a shade of pink. This can last a few weeks to several months. It is okay to apply makeup at this point, and our MedSpa can assist in finding the correct one. Sunscreen is a must during recovery and for the months ahead to protect your new skin. There is a little bit more post-care involved in a **full-field ablative erbium laser** like the ContourTRL, but the results tend to be worth the effort. #### Ablative Laser *Frequently Asked Questions* --- ## Restylane Silk **Link:** [Restylane Silk](https://www.oceandrivedermatology.com/injectables-fillers/restylane-silk/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/restylane-silk/index.html.md)) **Type:** Service **Description:** Restylane Silk is new to the Restylane portfolio. This filler is FDA approved for treatment of lip lines and volume loss in the lip itself. Contact us to schedule a consultation! ### Content **Restylane Silk** is a new addition to the Restylane portfolio in the United States. This filler is FDA approved for treatment of those dreaded lip lines and volume loss in the lip itself. This filler uses their proprietary NASHA gel. NASHA stands for “Non Animal derived Synthetic Hyaluronic Acid”. **Restylane Silk** is very similar to Restylane-L, but it has one very clever innovation. The hyaluronic particles in it are much smaller than the particles in Restylane-L. This allows for a slightly easier tissue interface after the filler is injected. Like Restylane-L, it does have the lidocaine numbing agent placed into the filler itself, allowing for a comfortable injection. This filler was designed to provide *subtle lip augmentation* while simultaneously reducing the dreaded lip lines. It is not intended to give extensive volumization to the lip region. Instead, it is designed to** maintain the natural anatomy of the lip while simultaneously improving volume**. This filler generates a mild degree of water absorption, as well as a very limited lift effect. We are beyond thrilled with the performance of this filler in our practice. While many women want the Angelina Jolie/Kylie Jenner look, just as many do *not*. Many women simply want to replace the volume loss that occurs over time, and *create a natural but full look to the lip*. For women looking to conservatively improve their lip appearance in Vero Beach **Restylane Silk** is the standard bearer in our opinion. The refined, limited molecular size of Restylane Silk provides a seamless interface between the lip anatomy and the filler gel, providing a true anatomic correction to the lip. Prior to Restylane Silk, [Juvederm Ultra XC Plus](https://www.oceandrivemedspa.com/injectables-fillers/juvederm-ultra-plus/) was the only consistent choice for lip volumization, and sometimes Juvederm can be too dramatic of a filler when it comes to Mom. With **Restylane Silk** Vero Beach patients now have access to a wonderful solution for men and women seeking a conservative, natural lip augmentation. We are very impressed by this filler, and use it avidly in our practice. As compared to other fillers, it is only being used in lips, as that is directly what it was designed for. **To date, most patients only require one tube, and no patient has required more than 2 tubes of product for lip correction.** --- ## Restylane Lyft **Link:** [Restylane Lyft](https://www.oceandrivedermatology.com/injectables-fillers/restylane-lyft/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/restylane-lyft/index.html.md)) **Type:** Service **Description:** Restylane Lyft is similar to Restylane-L, but it has a greater degree of viscosity making this filler a competitor to Radiesse Plus®. ### Content Restylane Lyft® is owned and distributed by [Galderma Aesthetics.](https://galderma.com/) It was previously known as Perlane, but has undergone a recent rebranding, named as **Restylane Lyft**. Restylane Lyft is similar to Restylane-L, but it has a greater degree of viscosity due to increased cross-linking. This cross-linking and increased viscosity makes this filler a competitor to Radiesse Plus® and [Juvederm Voluma](https://www.oceandrivemedspa.com/injectables-fillers/juvederm-voluma/). It has a good overall g prime, and does generate tissue lift. **Restylane Lyft is a hyaluronic acid based filler**, and as such, can be reversed with the use of Hyaluronidase. Despite its recent rebrand, this product has been on the market for over 10 years. From a clinical outcomes perspective, **Restylane Lyft is a safe and reliable product**. It serves as almost a “midpoint” filler between Voluma and Radiesse Plus®. While Radiesse Plus® offers the greatest degree of tissue lift, and while Voluma has the largest degree of water absorption of the midface fillers, Restylane Lyft offers a little bit of both. This filler, after injected, provides *a moderate lift with a moderate degree of water absorption*. As such, it can be effectively used to both **volumize and lift the midface**. Restylane Lyft® is a dosage independent filler, and it does last a minimum of 1 year. In our practice, we use Restylane Lyft in the following situations: - Patients with good overall cheek position and volume, but have depth and depression of the nasolabial folds. In these patients, we directly inject the nasolabial fold and the cheek directly above. This provides a very natural appearing result by lifting and filling the area. - Older patients with loss of brow volume with mild to moderate brow descent. With conservative placement of **Restylane Lyft** Vero Beach patients can be provided a nice, natural correction in this area. - We use this filler as an internal moisturizer, similar to Juvederm XC Ultra plus, in patients with mild cheek volume loss. By undertaking aliquot microinjections throughout the skin, we are able to both correct any volume loss while simultaneously moisturizing the deeper dermis. We carefully avoid injecting this filler in the lower eyelid and lip areas. The viscous nature of this filler can lead to development of lumpiness in these areas. Overall, **Restylane Lyft is an excellent filler product** with many “boutique” uses in facial rejuvenation. --- ## RHA Collection **Link:** [RHA Collection](https://www.oceandrivedermatology.com/injectables-fillers/rha/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/rha/index.html.md)) **Type:** Service **Description:** Smooth dynamic wrinkles and restore volume with RHA fillers at Ocean Drive Dermatology, designed to move naturally with your facial expressions. ### Content #### Resilient Fillers for Real Expression *RHA Collection at Ocean Drive Dermatology* The RHA Collection of dermal fillers is the next evolution in aesthetic injectables, designed to move with your face, not freeze it. At Ocean Drive Dermatology in Vero Beach, we offer RHA fillers for patients who want to smooth dynamic wrinkles and restore volume without sacrificing natural expression. Developed in Switzerland and FDA-approved in the U.S., RHA is the first and only resilient hyaluronic acid filler formulated specifically for facial movement. It’s ideal for treating fine lines, folds, and volume loss while maintaining a soft, dynamic, and natural look. #### RHA at a Glance - **Treatment Areas: **Designed for dynamic facial areas (laugh lines, cheeks, mouth, and more) - **Unique Formulation:** Made with resilient hyaluronic acid that adapts to movement - **Longevity:** Results last up to 15 months, depending on the product and treatment area - **Safety:** FDA-approved for the correction of moderate to severe facial wrinkles - **Recovery & Results:** Minimal downtime with immediate, natural-looking results #### Flex + Fill: The Future of Injectables *What Is the RHA Collection?* The RHA Collection is a family of advanced hyaluronic acid (HA) fillers developed to mimic your skin’s natural hyaluronic acid more closely than traditional dermal fillers. Through a gentle manufacturing process, RHA retains longer HA chains, resulting in a soft, flexible gel that adapts to your facial movements for refined, natural-looking results. There are currently four formulations in the RHA line: - **RHA Redensity™** – For fine, etched-in lines (like barcode lines around the lips) - **RHA 2** – For superficial lines and moderate wrinkles - **RHA 3** – For deeper lines and dynamic folds - **RHA 4** – For deeper volume replacement in the cheeks, jawline, and facial contours Each product is tailored for administration at different depths, making the RHA Collection a flexible tool for complete facial rejuvenation. #### Intelligent Design for Dynamic Faces *What Is the RHA Procedure Like?* Your RHA treatment at Ocean Drive Dermatology is designed to be comfortable, quick, and tailored to your unique facial anatomy. Here’s what to expect on the day of your procedure: #### Subtlety That Makes The Biggest Impact *What Concerns Does RHA Treat?* RHA is designed to treat the types of wrinkles and folds that appear with movement—smiling, laughing, or speaking—while preserving your facial expressions. It’s particularly well-suited for: - Smile lines (nasolabial folds) - Marionette lines - Fine “barcode” or “lipstick” lines around the lips - Cheek and midface volume loss - Jawline contouring - Dynamic wrinkles around the mouth, eyes, and chin #### Why Patients Love RHA *What Are the Benefits of RHA Fillers?* Unlike traditional fillers that can sometimes feel stiff or visible beneath the skin, RHA was developed to behave more like your body’s natural hyaluronic acid. This makes it especially effective in areas where movement and expression are constant. Patients choose RHA because it offers: - **Natural-looking results** that look beautiful in both motion and stillness - **Long-lasting improvement**, with results lasting up to 15 months - **Minimal downtime**, often with just a few hours of mild swelling or redness - **Smooth integration**, even in thin or delicate skin #### Designed for the Modern Face *Am I a Candidate for RHA Fillers?* If you’re noticing fine lines, deepening folds, or a loss of volume, but still want to look like yourself, RHA fillers may be an excellent option. It’s especially popular among patients who are new to fillers or want subtle, expressive results that don’t appear “done.” During your consultation at Ocean Drive Dermatology, we’ll evaluate your facial anatomy and goals to determine which RHA filler, or combination, is the best fit for you. #### Subtle, Swift, and Stunning *RHA Recovery* RHA treatments are quick, with minimal downtime. You may experience slight swelling, bruising, or redness at the injection site, but most side effects resolve within a few days. To get the best outcome: - Avoid alcohol, vigorous exercise, and heavy sun exposure for 24–48 hours - Refrain from rubbing or massaging the treated area - Follow any customized aftercare instructions from your provider Results are visible immediately and continue to refine over the first few days as swelling subsides. #### RHA *Frequently Asked Questions* --- ## Juvederm XC Ultra Plus **Link:** [Juvederm XC Ultra Plus](https://www.oceandrivedermatology.com/injectables-fillers/juvederm-xc-ultra-plus/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/juvederm-xc-ultra-plus/index.html.md)) **Type:** Service **Description:** Juvederm XC Ultra Plus is a hyaluronic acid filler. It is based on naturally occurring substances found in the cell walls of our skin, hair, and mucus. ### Content Of all the available fillers, the **Juvederm** family of products are easily the most recognized by our patients. Juvederm products that includes [Juvederm Voluma](https://www.oceandrivemedspa.com/injectables-fillers/juvederm-voluma/) and **Juvederm XC Ultra Plus** are produced and distributed by Allergan Inc®. Allergan has developed and maintained a strong direct to consumer (i.e., patient) marketing campaign including television advertisements, magazine and newspaper print, and a strong internet campaign. Juvederm XC Ultra Plus, like [Restylane](https://www.oceandrivemedspa.com/injectables-fillers/restylane/), is a hyaluronic acid dermal filler. It is based on a naturally occurring substance found in the cell walls of our skin, hair, and mucus membranes. Juvederm XC Ultra, like Restylane-L, does have lidocaine inside of its gel carrier, which improves patient comfort during the procedure. Juvederm XC Ultra Plus is the same parent molecule of Juvederm XC Ultra, but it has a greater degree of “molecular cross-linking”. Juvederm XC Ultra has 6% concentration of cross-linking, while Juvederm XC Ultra Plus has 8% concentration of cross linking. This increased cross-linking allows Juvederm to be much more durable. This durability translates into longer lasting results in our experience. **Juvederm is the undisputed heavyweight champion of all dermal fillers** in its ability to absorb water and provide effacement of moderate and deep wrinkles. Pound for pound, Juvederm has the greatest degree of hydrophilic activity, or the ability to absorb water, as compared to Restylane, [Radiesse](https://www.oceandrivemedspa.com/injectables-fillers/radiesse/), [Belotero](https://www.oceandrivemedspa.com/injectables-fillers/belotero/), etc. Juvederm is a very safe, very reliable filler that offers patients the opportunity to markedly volumize their face. Juvederm also serves a secondary role in that, because of its ability to absorb water, it serves over time to maintain moisturization of the deep dermis for months after injection. While the company cannot scientifically say that this product improves skin water volume, most master injectors agree that Juvederm does offer the skin excellent long-term secondary moisturization, especially in patients that use the product on a regular basis. While Juvederm XC Ultra Plus is the undisputed champion at water absorption, it also has the distinction of offering next to zero tissue lift. In other words, unlike other fillers like Restylane and [Radiesse](https://www.oceandrivemedspa.com/injectables-fillers/radiesse/), the Juvederm XC family cannot provide a lifting effect to the soft tissue it treats. It has a very low g’ value, with poor overall cohesivity and viscosity. As such, it cannot latch onto tissue and pull it up. However, it can act as an internal “water beacon”, and draw hydration into the deep dermis and subcutaneous planes, offering outstanding skin rejuvenation. Juvederm XC Ultra Plus is incredibly useful in aesthetic plastic surgery of the face. At Ocean Drive Medspa in Vero Beach **Juvederm XC Ultra Plus** is used routinely in the following situations: - Patients who want Kylie Jenner-looking lips. Look no further than Juvederm. Because of its incredible ability to absorb water, none of the other fillers can produce a pillowy looking lip like Juvederm XC. For patients that are looking to increase lip volume and produce a “wow the crowd” effect to their lips, Juvederm is again the undisputed champion. If you want Angelina Jolie lips, you want Juvederm. - Deep inset marionette lines. For patients that have grooves in their lower face from their lips to their chin, Juvederm is an outstanding choice. Again, its ability to bring fluidity into a region allows for effacement and minimization of deep marionette lines. - Nasolabial folds in patients without cheek descent. Some people have an empty appearing nasolabial fold even if their cheek is in perfect position. In this case, we will inject Juvederm into the nasolabial fold directly to provide a smooth, harmonious juncture between the cheek, nose, and lip area. **Juvederm XC Ultra Plus is a very versatile filler**, and can be used throughout the face. However, there are a number of areas where this filler should not be used: - The lower eyelid. When Juvederm is injected into the lower eyelids, it will regularly produce an unsightly lumpiness to the skin. Avoid use of this filler in the lower eyelids. - The cheek in patients who have a droopy cheek. Juvederm will not lift tissue. It will only volumize the area in which it is injected. As such, in patients who want a cheek lift, Juvederm XC Ultra will not effectively lift the area, but it will make you look like a chipmunk if you inject too much. For patients with cheek descent, we commonly use Radiesse Plus, or Voluma, to elevated the cheek as both of these injectables provide tissue elevation. --- ## Xeomin **Link:** [Xeomin](https://www.oceandrivedermatology.com/injectables-fillers/xeomin/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/xeomin/index.html.md)) **Type:** Service **Description:** Xeomin has the same parent molecule as Botox, but unlike Botox, it begins working faster, for more immediate results! ### Content Xeomin® is a neuromodulator that is in the same class as [Botox®](/injectables-fillers/botox/). **Xeomin is FDA approved** for aesthetic use in the human face including the treatment of moderate to severe wrinkles in the glabella. It is also has therapeutic indications and is the first-line therapy for blepharospasm. Both Botox and Xeomin can be used throughout the face in a safe fashion. Most commonly, both are utilized in the forehead, around the eyes (crows feet), around the lip lines, and along the neck. Bruising is uncommon, and swelling resolves within an hour. Each of these treatments are common “lunchtime” interventions. There is no mistaking the brand presence of Botox, but also consider that Botox was introduced back in the 1980’s. There have been a number of innovations in neuromodulation since the landmark discovery of Botox. **Xeomin is the next step in the neuromodulator market**. **It provides the purest form of neuromodulation on the market, spares the possibility of preservative allergy/rejection, and acts faster while lasting just as long as Botox. Welcome to the next level of neuromodulators.** #### Xeomin®is Uniquely Purified **Xeomin** is the only highly purified neurotoxin on the market that uses a patented process called, Xtract Technology. Xeomin was created and is produced by Merz Aesthetics, a 112 year pharmaceutical company that focuses on nothing but aesthetic products. Their scientists developed the Xtract Technology to remove all of the unnecessary proteins away from the active toxin molecule. Their goal was to achieve an end-product that was as pure as possible. That makes sese, right? Why would anyone want impurities in a product that they would inject into their face? If they could minimize those impurities, wouldn’t they achieve a better purifies toxin than other available products on the market? So, using their special Xtract Technology, Merz is able to eliminate all the unecessary proteins that can result in immunogenicity and dose creep. And, in a double-blind patient satisfaction study, **Xeomin had a 93% satisfaction rate at month four** and that’s 3% higher than Botox. In a world where people are more conscious than ever about what they put in their bodies, for our patients in Vero Beach **Xeomin** is a great option. #### Xeomin® vs Botox® **Xeomin has the same parent molecule as Botox**, but unlike Botox, it begins working within 1-2 days rather than over 10-14 days. This is one of the main innovations that Xeomin brings to the table. The reason why it acts faster is because unlike Botox, Xeomin does not have a preservative protein. When Botox is injected, the botulinum molecule must free itself from its preservative protein, and then it is free to bond with the facial muscles. It bonds to the muscle, and the muscle relaxes. Once the muscle relaxes, the wrinkles fade away. Again, with Xeomin, there is no preservative protein. **Xeomin is immediately available to the muscle receptors**. It starts to bind immediately, so after **Xeomin** Vero Beach patients will notice reduced wrinkling at a markedly faster rate, and at a steadier pace. No preservatives in the mixture allows for a higher activity of the active molecule following injection. There are also significant differences in how Botox and Xeomin feel to the patient. As a patient myself, I have received both Botox and Xeomin. **Xeomin feels much lighter and more natural than Botox**, in my opinion. With Xeomin, the wrinkles disappear, but I still maintain normal motion. Botox tends to be a bit stronger, in my opinion. It has a heavier feeling on the brow, and it tends to limit muscle movement to a greater degree. As we always preach at Ocean Drive Dermatology, one solution does not fit everyone. We use Botox in patients who want a more dramatic result from their neuromodulator. We use Botox in patients who do not mind that heavy sensation, and in patients that want to curtail muscle movement. For patients that want less muscle movement, and more natural facial function, we have found that Xeomin offers a superior result to those clients that want to maintain muscle function but limit the amount of facial lines they see in the mirror. --- ## Juvederm Voluma **Link:** [Juvederm Voluma](https://www.oceandrivedermatology.com/injectables-fillers/juvederm-voluma/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/juvederm-voluma/index.html.md)) **Type:** Service **Description:** Juvederm Voluma is designed to treat the sagging midface, or cheek area. Voluma was developed by Allergan to specifically treat the midface, or cheek region. ### Content **Juvederm Voluma** is the newest addition to the Juvederm family in the United States. This dermal filler was developed by [Allergan®](https://allergan.com/) to specifically treat the midface, or cheek region. Unlike Juvederm Voluma, [Juvederm XC Ultra](https://www.oceandrivemedspa.com/injectables-fillers/juvederm-ultra-plus/) is NOT designed to lift the sagging cheek area. Rather, it is intended to fill areas of moderate to deep wrinking by having a low cohesivity while simultaneously having a high degree of water absorbancy. When Juvederm XC is injected into the midface for lift, men and women tend to look unnatural, resembling the “chipmunk cheek” look. Because of this, Allergan® developed and launched a second type of filler known as **Juvederm Voluma**. **Juvederm Voluma is designed to treat the sagging midface, or cheek area**. It is FDA approved for treatment of malar (aka cheek) atrophy. This novel molecule does not rely upon cohesivity and g prime to achieve lift like other dermal fillers. In the case of Voluma, the dermal filler is a gel which has randomly sized cross-linked molecules that are based on hyaluronic acid. Essentially, the random sizes of the hyaluronic acid molecules interact in the deeper facial planes with your soft tissue to create lift. This is recognized as a different mechanism than all of the other dermal fillers available, and it has some strengths and weaknesses. The big strength of **Voluma** is that it does have a hyaluronic acid as its foundation molecule. Because of this, the effects of Voluma can be reversed and the filler itself can be removed with the injection of an enzyme called hyaluronidase. To be clear, this means that if you don’t like your Voluma injection, your injector can literally remove it with a second injection. It typically takes about 3-5 days for the product to completely dissolve, but it will dissolve. The other great strength of Voluma is that as a hyaluronic acid based filler, it does integrate seamlessly into soft tissue, and when appropriately injected by a experienced injector, it does create natural, beautiful results. Also, Voluma has been shown to last up to two years in duration following injection, although the length of longevity is variable amongst individual patients. The liability of Voluma, however, is also due to its molecular makeup. As this molecule does rely upon a random molecular size to generate lift, the amount of lift you receive with Voluma appears to be very dose dependent. In other words, patients who use low volumes of Voluma (less than 3cc) do not typically receive the same degree of lift they would receive with Radiesse Plus, or [Restylane Lyft](https://www.oceandrivemedspa.com/injectables-fillers/restylane-lyft/). Since both the Restylane and Radiesse products rely upon individual molecular lift, the presence of a single molecule or a million molecules will create lift. Voluma, on the other hand, requires a baseline volume to be injected to get the maximal lift effect, and the amount needed can vary widely from patient to patient. As stated previously, Voluma is FDA approved for treatment of sagging cheeks. During the FDA clinical trials, patients appeared to get the best results using, on average, 5.6 milliliters of Voluma. When used at this volume, patients did get a significant lift effect, and the product lasted at least 18 months in most cases. Our practice has had a similar experience as the FDA trials. We have found that patients are most content with Voluma when it is used in higher doses, such as 5-7 milliliters. When used in this volume, this filler can and does provide outstanding lift, and will last over 18 months. However, many patients do not want that degree of filling, as one of our most common concerns from patients seeking dermal therapy is “looking overdone”. Lastly, many of our Vero Beach **Juvederm Voluma** patients do not require such a large degree of correction, especially men and women under the age of 55. **We find Voluma to be best suited for older patients who require significant correction. In other words, we try to use Voluma in patients who need significant volumization, usually over 6 milliliters injected. In this scenario, the product can last up to two years, and offer an excellent result. Overall, this is a very safe filler that offers effective volumization to the midfaces, especially in higher injection volumes (greater than 6 milliliters).** --- ## Male and Female Pattern Hair Loss **Link:** [Male and Female Pattern Hair Loss](https://www.oceandrivedermatology.com/skin-conditions/male-and-female-pattern-hair-loss/) ([md](https://www.oceandrivedermatology.com/skin-conditions/male-and-female-pattern-hair-loss/index.html.md)) **Type:** Service **Description:** Male and female pattern hair loss is very common and affects tens of millions of patients. The medical term for this type of hair loss is called androgenetic alopecia. ### Content **Male and female pattern hair loss** is very common and affects tens of millions of patients. The medical term for this type of hair loss is called androgenetic alopecia. It is typically recognizable in men by a **balding patch on the crown of the scalp or as receding hair line**. Most men in the 30s, sometimes younger have already noticed hair loss or thinning of the hair. Women do not usually end up with patches of hair loss, but more so a significant thinning of hair. Typically, we see this post menopause. Both male and female pattern hair loss tends to be genetic, but we believe hormones also contribute to hair loss. *Genetics and hormones are not the only cause of hair loss*, but the most common. Other causes of hair loss could be autoimmune, medications, illness, and even tractional alopecia which is usually caused by hairstyles that pull the hair tight. There is not a cure for hair loss. The good news is that there are plenty of **treatments for male and female pattern hair loss** that have a proven track record. Ultimately, each patient responds in their own way to treatment. Sometimes the treatments are successful at growing hair and sometimes the treatments are only successful in stopping the progression of the hair loss. Most times, multiple treatments are needed for the best long-term results. --- ## Dysport **Link:** [Dysport](https://www.oceandrivedermatology.com/injectables-fillers/dysport/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/dysport/index.html.md)) **Type:** Service **Description:** Don’t let frown lines on your face speak for you! You will see remarkable results within days that will continue to improve over time. #### A Quick Recovery *Halo Hybrid Fractional Laser Recovery and Aftercare* Unlike traditional [laser resurfacing](/laser-treatments/ablative-laser/), which can include a recovery time of up to a month, recovery from a Halo Laser procedure is about one week. We provide all your aftercare products as part of your Halo laser package, including a moisturizer, face wash, and sunscreen. We only use physician-grade post-care products from our MedSpa. After the first 24 hours have passed, the treated skin will begin to slough off as new healthy skin comes to the surface. You may want some social downtime for a few days as your skin is regenerating, but you will be able to resume normal daily activities as desired. We also include a Red LightStim treatment 24 hours after your procedure to reduce inflammation and speed up your recovery. The aggressiveness of your treatment dictates the type of recovery you will have. Generally, patients are fully recovered in five to seven days. #### Halo Hybrid Fractional Laser *Frequently Asked Questions* --- ## Moles (Melanocytic Nevi) **Link:** [Moles (Melanocytic Nevi)](https://www.oceandrivedermatology.com/skin-conditions/melanocytic-nevi/) ([md](https://www.oceandrivedermatology.com/skin-conditions/melanocytic-nevi/index.html.md)) **Type:** Service **Description:** When we talk about melanocytic nevi, we are talking about benign (normal, non-cancerous) moles on the body. Contact Ocean Drive Dermatology today to find out more! ### Content #### Where did all these moles come from? When we talk about **melanocytic nevi**, we are talking about benign (normal, non-cancerous) moles on the body. You were either born with these moles (congenital nevi) or developed them sometime after you were born (acquired). You may have just 4 or 5 nevi on your body or you may be someone with hundreds scattered from head to toe. We know that heredity plays a role in the development of nevi, as well as being fair skinned. If your mom, dad, brother or sister have a lot of moles on their body then you might as well. How much sun exposure you get when you are younger can also play a role in how many moles you have on your body. **It’s not uncommon to get new moles up to about 35 or 40 years old**. #### Is this where melanoma comes from? Yes and no. We know that the vast majority of **benign moles** remain benign throughout their entire life. *Sometimes those benign nevi can change into skin cancer*. Having hundreds of nevi on your body does increase your risk for melanoma. Keep in mind though that the majority of melanomas are from new moles, not existing moles and from long-term sun exposure. #### Atypical Moles or Nevi **Atypical nevi come from acquired melanocytic nevi**. Sometimes a mole is biopsied and removed because it looks atypical or abnormal, but the biopsy results come back with no skin cancer. However, the results show atypical nevi. Does atypical mean skin cancer or pre-skin cancer? No; not necessarily. Atypical nevi are typically graded on the following scale: mildly atypical; moderately atypical; moderately-to-severely atypical; and severely atypical. Although technically any atypical mole could turn into skin cancer, specifically melanoma, studies show that the vast majority do not do this. Typically, we surgically remove moderately-to-severe or severely atypical moles as that severity has a higher likelihood of transitioning into a melanoma. Monitoring mild and moderately atypical moles is an option. All patients and diagnoses are unique and thus these are general guidelines discussed here. #### Ugly Duckling There are numerous other types of melanocytic nevi, but the most important thing to know is what to look for in case you think the mole is concerning. Always look for the “ugly duckling”. If all of your moles are brown or light brown and you get a new one that is jet black that would be the “ugly duckling”. If all of your moles are about 5 mm large and now you have one that is 7 mm that would be the “ugly duckling”. If you find an “ugly duckling” the best thing to do is make an appointment to be seen. We will look at your mole, usually under dermoscopy (a fancy magnifying glass), and compare it to your other moles on your body. There are typically three outcomes from evaluation of a mole of concern. We might tell you the mole is non-cancerous. We may recommend re-evaluating in 3 months. In this case, we usually take photographs of the mole to compare when we see you back in 3 months. Finally, we might recommend a simple procedure called a skin biopsy. This is a process that takes about 5 to 10 minutes and allows us to sample the mole and send it to the lab to determine if there is anything abnormal. The results determine our next step. #### Skin Cancer Screening If you have never had a total body skin exam, we recommend getting one. Then you will have a baseline of what is normal in regards to your moles. --- ## BBL Laser **Link:** [BBL Laser](https://www.oceandrivedermatology.com/laser-treatments/broadband-light-bbl/) ([md](https://www.oceandrivedermatology.com/laser-treatments/broadband-light-bbl/index.html.md)) **Type:** Service **Description:** Broadband light treats sun-damaged skin without downtime. Five treatments are typically necessary, with a three-week break between each session. Even after the first treatment, you will notice that your skin looks and feels better. Sunspots will lighten, and the skin will appear smoother and softer with continued treatments. #### Gentle Skin CareContinued Improvements We recommend a gentle skincare routine while your skin recovers from treatment. One of the most important things to do is to wear sunscreen and a hat. There is no actual downtime associated with Broadband light. You should expect some redness and blotchy-looking skin post-treatment. Sometimes a sunburn-like sensation is noticed that usually goes away within the first 24 hours. Occasionally, there is some swelling. If the treatment was targeting sunspots or hyperpigmentation, you might notice these spots get darker. They will slough off the skin by day 14 when treating the face; longer for other body areas. Makeup can be worn. #### Broadband Light BBL *Frequently Asked Questions* --- ## ClearSilk Laser **Link:** [ClearSilk Laser](https://www.oceandrivedermatology.com/laser-treatments/clearsilk-laser/) ([md](https://www.oceandrivedermatology.com/laser-treatments/clearsilk-laser/index.html.md)) **Type:** Service **Description:** A ClearSilk Laser procedure at Ocean Drive Dermatology in Vero Beach is your real-life photo filter! About 80 percent of non-melanoma (squamous cell and basal cell) skin cancers will be a basal cell carcinoma. As expected, those with fair skin and lots of cumulative sun damage are at higher risk. Vacations at the beach, sunburns when a teenager, tanning beds, and walking the dog without sunscreen can contribute to the development of basal cell skin cancer. Just think of all the times you are in the sun without sunscreen. Those times are part of the cumulative UV exposure that can lead to BCC. If you were to develop a skin cancer in your lifetime this is the one you want. The risk of spreading is very low. Both young and old are at risk for **basal cell skin cancer**. Although we typically diagnosis this in the older population it is not unheard of to treat someone in their 20s for this. Sunburns, fair skin, and the tanning bed fad may be at fault for the younger generation developing these. Also, there are a lot of medications that make people more sensitive to sunlight. Blood pressure medications, antibiotics, and even acne treatments can make the skin more sensitive to UV light. Just like [squamous cell skin cancer](/skin-conditions/squamous-cell-carcinoma/), those that are immunosuppressed or have had long term radiation therapy are at higher risk for BCC, regardless of sun exposure. There are several type of basal cell skin cancer, but we will focus on the two most common. Nodular is the most common form of BCC accounting for about 50-60 percent. These can occur anywhere on the body that has had excess sun exposure. They tend to look like pearly dome shaped bumps and bleed easily. Nodular BCCs tend to ulcerate in the middle. Superficial BCC can also occur anywhere on the body. Unlikely the “pearly papule” of a nodular BCC these tend to be flatter and shinier or even sometimes scaly. Also, they don’t tend to bleed like the nodular variant. There are other types of BCC called morpheaform, micronodular, and infiltrative. These three tend to be more aggressive so some treatment options that exist for a superficial or nodular basal cell would not be appropriate for these types. Treatment for BCC is relatively straight forward. The same options that are available for squamous cell skin cancer are available for basal cell skin cancer. There are several treatment options for BCC based on location of the skin cancer (nose vs forearm) and type (superficial vs nodular). Treatment could include excision, Mohs micrographic surgery, electrosurgery (ED&C, electrodesiccation and curettage), radiation therapy, and topical therapies such as fluorouracil (Efudex). Photodynamic therapy (PDT) or “Blue Light” is also an option for superficial basal cell skin cancer with good cure rates. The best treatment is not the same for every person and every type of basal cell skin cancer. After diagnosis of a BCC we will discuss with each patient the risks and benefits of each recommended treatment option. Once the skin cancer is removed or otherwise treated it is extremely important to have good follow up. Like all cancer, skin cancer can recur in the same treated area or develop in a new area. **We recommend a total body skin exam every six months for two years after the diagnosis of a basal cell skin cancer**. If no other skin cancers are found during that time period we then recommend yearly total body skin examinations. Possibly more important is the need to look over your body once a month at home. Trust your instincts. If a spot does not heal, bleeds, or somehow is different from your other moles you should come in for an office visit sooner. --- ## ClearV Laser **Link:** [ClearV Laser](https://www.oceandrivedermatology.com/laser-treatments/clearv-laser/) ([md](https://www.oceandrivedermatology.com/laser-treatments/clearv-laser/index.html.md)) **Type:** Service **Description:** Say goodbye to hiding unwanted veins under layers of makeup. Our Vero Beach **ClearV Laser** providers will consult with you about your skincare needs and help rid you of visible vessels, broken capillaries, spider veins, vascular lesions, skin discoloration, rosacea, and more. This treatment requires no downtime, and you can return to your normal activities as soon as you leave the office. **In addition, you will notice veins are not as visible after just one treatment, and your skin will look even more flawless over time.** #### Confidence withClear Skin *What is ClearV Laser?* The **ClearV** is an Nd:YAG 1064 laser that is the gold standard for removing vascular lesions (spider veins, telangiectasias, port wine stains, leg veins, and periorbital veins) on the face and body. We use this laser in conjunction with [Broadband Light](/laser-treatments/broadband-light-bbl/) and [Broadband Light HERO](/laser-treatments/bbl-hero/) to reduce the redness or erythema that many rosacea sufferers contend with daily. The reason why the 1064 is considered the standard to remove facial vessels is that it has a strong absorption of something called oxyhemoglobin. This allows the laser to *selectively heat veins to remove or make them less visible* while not harming the skin. Therefore, **we can treat darker skin types without concerns** about changing their skin's pigment color. #### Shine Your Light *What are the benefits of ClearV?* Too often, we find ourselves hiding the imperfections in our skin under layers of makeup or strategically using clothing to hide broken veins and other problem areas on our skin. At Ocean Drive Dermatology in Vero Beach **ClearV Laser** treatments can help you gain back the confidence you need to walk into any room and shine like the light you are. The ClearV Laser illuminates the problematic areas underneath your skin during your treatment. Then, it gently delivers laser pulses to eliminate them and **give your skin a more smooth and glowing appearance**. You will notice improvements in your skin immediately after the first treatment, and your results will improve even more over time. #### Experts Here for You *Your ClearV Laser Consultation* During your complimentary consultation, your medical provider will listen to your skin care needs and collaborate with you to determine the appropriate treatment to achieve optimal results. The exact cause of Bell’s palsy is not known however, herpes simplex virus activation is the likely cause of in most cases. **The annual incidence rate is between 13 and 34 cases per 100,000 people**. There is no race, geographic, or gender prediction, but the risk is three times greater during pregnancy, especially in the third trimester or in the first postpartum week. The increased risk of Bell’s palsy associated with pregnancy, which is most marked in the third trimester and the first postpartum week, may be caused by pregnancy-related fluid retention leading to compression of the nerve or perineural edema. Other potential etiologic factors include hypercoagulability causing thrombosis of the vasa nervosum and relative immunosuppression in pregnancy. Several studies have found an association of Bell’s palsy with preeclampsia, again suggesting extracellular edema as the mechanism. Patients with **Bell’s palsy** typically present with the sudden onset (usually over hours) of unilateral facial paralysis. Common findings include the eyebrow sagging, inability to close the eye, disappearance of the nasolabial fold, and drooping at the affected corner of the mouth, which is drawn to the unaffected side. Decreased tearing, hyperacusis, and/or loss of taste sensation on the anterior two-thirds of the tongue may help to site the lesion in the fallopian canal, but these findings are of little practical use other than as indicators of severity. **Diagnosis of Bell’s palsy is typically clinical**. Blood work and imaging studies often yield little in the diagnosis. However, blood work and imaging studies are often performed to rule out other possible causes of the facial paralysis. Lyme disease testing is strongly recommended for adults with acute-onset facial palsy when there is the possibility of exposure in Lyme-endemic areas. Upon the initial diagnosis of Bell’s palsy, it is important to treat with glucocorticoids to help improve the likelihood of recovery. *Most patients recover completely within several months to a year*, however up to one-third have unfavorable functional and cosmetic outcomes due to chronic facial weakness. For such patients, surgical procedures and botulimun toxin injections ([Botox](/injectables-fillers/botox/), [Xeomin](/gallery/injectables-fillers/xeomin/), [Dysport](/injectables-fillers/dysport/)) are available to help mitigate long-term sequelae. Synkinesia and contractures of the face develop in approximately 15% of patients as a late complication of Bell’s palsy due to disorganized and misdirected nerve regrowth on the side of injury. **Synkinesia** refers to involuntary contraction or twitching of part of the facial musculature upon voluntary facial movement. Most synkinesia after Bell’s palsy is mild and may not be noticed by the affected individual. **More severe or bothersome synkinesia can be treated with botulinum toxin injections**. Botox can be used to selectively inhibit muscle contractility. Common facial muscles targets include the orbicularis oculi, depressor anguli oris, depressor labii inferioris, mentalis, and platysma. The focus of treatment is to help balance the facial muscles to give the face a more symmetrical appearance. In order to do so, it requires a great deal of understanding of not only the muscle anatomy but the type of neuromodulator used. During the initial consultation a thorough evaluation of muscle strength and symmetry of the face is performed. I then map out the injection points, type of neuromodulator I use, and amount of neuromodulator I use for each injection point. Often I will end up with several syringes but each will only have 2-6 units in each. I perform plenty of injections of neuromodulators for cosmetic reasons but nothing compares to the feeling of helping someone regain confidence and restore the balance to their face. If you are have been affected by Bell’s palsy I urge you to please schedule a complimentary consultation and take the first steps to achieving a balanced face. --- ## Laser Treatments Overview **Link:** [Laser Treatments Overview](https://www.oceandrivedermatology.com/laser-treatments/overview/) ([md](https://www.oceandrivedermatology.com/laser-treatments/overview/index.html.md)) **Type:** Service **Description:** The most effective method of direct skin rejuvenation is laser therapy. Not all PRP is created equal and we utilize a system that has been proven to have a platelet recovery of more than 80% with a concentration of more than 1.6 times that of the baseline. Although there is currently no agreed upon consensus of the correct concentration of platelets there are studies showing that too high a concentration of platelets can have negative effects. This is why we have chosen a PRP system that is based on data, journal published studies, and real-life experience. At Ocean Drive Medspa in Vero Beach **PRP hair restoration** is safe, effective, quick, and with very little discomfort. Initially, you will have a complimentary consultation with either our Board-Certified Plastic Surgeon, Alan Durkin, MD or our Dermatology Physician Assistant, Nick Woltjen, PA-C. Obtaining PRP takes about 10 minutes in a completely closed system. The closed system means that from blood draw, to centrifuge, to injections your blood and platelets are never exposed to the environment. This is yet another reason why our system is safe and effective. Step 1: One of our nurses will obtain only one tube of blood from either your left or right arm. You will let us know which arm you prefer or if you know one arm is easier to draw blood from or not. This is not any regular tube that we are using. This tube contains special buffers and gel separatory technology to guarantee a correct concentration of PRP. Step 2: We then put the tube in a special centrifuge to separate the platelets. This centrifuge spins for a set number of minutes at a very specific revolution to give us high quality PRP. Step 3: The PRP is ready for injection. We utilize Pro-Nox Nitrous Oxide with all of our PRP injections for hair loss. Pro-Nox delivers a mixture of 50% oxygen and 50% nitrous oxide. This device is a 100% patient-controlled form of inhaled anesthesia. Simply breathe deep and in seconds there will be immediate relief of pain and anxiety. We are proud to offer **PRP Hair Restoration** to our patients at no additional charge as part of all of our PRP hair loss packages. **The Pro-Nox anesthesia leaves your body in just a few minutes after treatment. You can continue your day immediately after treatment.** --- ## Botox® **Link:** [Botox®](https://www.oceandrivedermatology.com/injectables-fillers/botox/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/botox/index.html.md)) **Type:** Service **Description:** All neuromodulators, including Botox, remove facial wrinkles by temporarily and partially relaxing the muscles of the human face. ### Content #### Botox® Injectionsin Vero Beach, Florida *BOTOX® Injections at Ocean Drive Dermatology* Botox Cosmetic (onabotulinumtoxinA) is more than a wrinkle remover. It is a nonsurgical method of graceful aging. **Botox** is a prescription medicine that is injected into facial muscles. Both men and women may receive injections to *soften the appearance of moderate to severe frown lines* on the face. At [Ocean Drive Dermatology](/), we only buy our Botox directly from the makers, Allergan Inc., to ensure that we receive pure, fresh (unexpired), and undiluted products. We solubilize the product according to Allergan guidelines, and we don’t dilute the product to make it “last longer.” We also store Botox according to the manufacturer’s recommendations. To ensure that proper storage temperatures are maintained, we have electronic thermostats that monitor the temperature in the storage unit 24 hrs a day, seven days a week. We are alerted to any fluctuations in temperature that could result in the degradation of the product. Thus, you are ensured of getting injections of stabilized and active Botox products. At Ocean Drive Dermatology, our philosophy is to provide a comprehensive improvement to your facial aesthetics by **using Botox to improve brow position and eyelid aesthetics**. As a provider who has cross-trained in both Plastic Surgery and Dermatology, we routinely perform surgical brow lifting and eyelid (blepharoplasty) procedures. We have a deep and comprehensive understanding of the aesthetics of the human face and apply it to every injectable treatment we provide. When it comes to **Botox** Vero Beach providers at Ocean Drive Dermatology use this global understanding to separate our results from other Botox specialists. #### Botox® Revolution *What is Botox?* **Botox is one of the great advancements in plastic surgery** over the last 20 years. Botox is a purified form of the neurotoxin protein found in the bacteria called Clostridium Botulinum. It was developed and distributed by Allergan Inc., and its initial uses were for the treatment of blepharospasm, which is a condition of constant eyelid twitching. It was also used for the treatment of esophageal achalasia, a disorder where the lower esophagus is unable to relax, leading to an inability to eat properly. Upon injection of Botox, the affected muscles stop contracting, which temporarily cures blepharospasm and achalasia of the esophagus. During the treatment of blepharospasm, it was noted that it not only stopped eyelid twitching but it also reduced the appearance of periorbital wrinkles, also called crow’s feet. After its initial successes, **Botox** was then resubmitted to the FDA for aesthetic usage in the human face. The FDA subsequently approved **Botox for use as a temporary wrinkle remover**. Since its approval for plastic surgery usage, *Botox has rapidly become the most sought-after plastic surgery treatment in the country*, with over six million treatments delivered annually in the United States alone. #### How does Botox work on wrinkles? All neuromodulators remove facial wrinkles by temporarily and partially relaxing the muscles of the human face. This relaxation effect leads to a **marked reduction in facial lines and wrinkles** because facial muscles attach directly to the skin. **Botox is most effective in the glabella** (the 11’s between your eyebrows), the horizontal forehead lines, and on your crow’s feet, which are those small lines surrounding your lateral eyes. Beyond its ability to reduce facial wrinkles, *Botox injections can provide a lifting effect to your eyebrows* and comprehensively improve the upper 1/3 of the face. #### Safe and Effective *What are the benefits of Botox?* The use of Botox neurotoxins is completely safe, requires ZERO downtime post-injection, and is completely effective in removing what we call ‘animational rhytids,’ the medical term for skin wrinkles caused by repetitive muscle contraction, aka muscle wrinkles. This means that it will work on areas where there is constant muscle motion; the forehead, glabella, and around the eye areas. However, **Botox works on the outer area of the eye** and not the inner area of the eye. **Wrinkle reduction of Botox lasts anywhere from 2-4 months with an average of 3 months length.** #### Firm and Fresh *Botox Recovery and Aftercare* Though there is no downtime after receiving Botox, there are best practices to follow post-procedure. Do not exercise or lie down for at least three hours. Avoid saunas, hot tubs, and tanning booths for at least four hours. These will raise your blood pressure and might lead to bruising. Do not get a head or facial massage for 24 hours, and avoid rubbing your face at the injection site. Your Ocean Drive Dermatology professional will give you a complete list of do’s and don’ts after your procedure. #### Botox® *Frequently Asked Questions* --- ## Contact Dermatitis **Link:** [Contact Dermatitis](https://www.oceandrivedermatology.com/skin-conditions/contact-dermatitis/) ([md](https://www.oceandrivedermatology.com/skin-conditions/contact-dermatitis/index.html.md)) **Type:** Service **Description:** Just like eczema or atopic dermatitis; allergic contact dermatitis causes red, itchy, and sometimes oozing skin. ### Content Just like eczema or atopic dermatitis; allergic contact dermatitis causes red, itchy, and sometimes oozing skin. Also, it can just as easily become infected. Looking at the skin it is sometimes hard to differentiate between all these red rashes. The history of the rash is extremely important to help differentiate if this is eczema or allergic contact dermatitis. Those with eczema or atopic dermatitis tend to have a life-long history of have red itchy rashes that come and go. On the other hand, those with allergic contact dermatitis tend to have a new onset rash. Maybe you used a new eyeliner and now your eyelids are red and itchy. The interesting thing with allergic contact dermatitis is that you could develop an allergy to a product you have been using your entire life. This sometimes makes it harder to figure out what is causing the rash. *Ocean Drive Dermatology now provides the T.R.U.E Test which tests for the top 35 most common topical allergens.* A detailed history can sometimes pinpoint the cause of the allergy, but sometimes further investigation is needed. Allergy patch testing of the skin can help identify the offending allergen. Allergy patch tests are applied to the skin and left on for 2-3 days. Any area of the test that you develop a red spot is a potential allergen. Once the allergen is identified you will need to avoid all products containing that chemical. Usually once allergen avoidance is initiated the rash calms down and goes back to normal skin. However, sometimes the immune system is in “overdrive mode” and it could take months to calm down even after avoiding the allergen. Some people are allergic to numerous allergens at one time and others just one or two. Treatments are similar to eczema and atopic dermatitis. Topical steroids creams, non-steroid creams, oral steroids, and in rare cases nonsteroidal immunosuppressive medications such as methotrexate or mycophenolate could be utilized. --- ## Evolysse **Link:** [Evolysse](https://www.oceandrivedermatology.com/injectables-fillers/evolysse/) ([md](https://www.oceandrivedermatology.com/injectables-fillers/evolysse/index.html.md)) **Type:** Service **Description:** Smooth wrinkles and restore facial volume with Cold-X™-powered Evolysse fillers at Ocean Drive Dermatology in Vero Beach. Examples of some benign growths that respond well to cryotherapy are [warts](/skin-conditions/wart-removal/), skin tags and seborrheic keratoses, the latter of which patients here me call “barnacles of wisdom” during skin cancer screenings. An example of a pre-malignant growth are actinic keratoses. [Actinic keratoses](/skin-conditions/actinic-keratosis/) are pre-malignant spots that show up on sun exposed areas of the skin and can sometimes turn into squamous cell carcinoma. When there are just a few actinic keratoses on the body **cryotherapy is a good option**. However, when there are numerous, we typically recommend other treatment options. At Ocean Drive Dermatology in Vero Beach **skin biopsies** are typically done in office and are relatively painless and completed quickly. #### Types of Skin Biopsies There are two main types of skin biopsies: shave biopsy and punch biopsy. #### Skin Biopsy Procedure Steps *What happens during the biopsy?* The very first thing that happens is the procedure is explained to you from start to finish and then consent forms are signed. The entire procedure, although it may seem lengthy, takes about 5 minutes. #### Possible Biopsy Complications and Risks *What are possible complications with a biopsy?* Typically, there are three main items to worry about with a biopsy and keep in mind that this is a very low risk procedure. - Bleeding: Blood loss is nonexistent or very small during a shave or punch biopsy. We will stop any bleeding prior to you leaving the office. This may include using Drysol, Pressure Bandage, Electrocautery, or Silver Nitrate. - Scarring: There will be some type of scar post-procedure. This is usually very minimal, and we can recommend different treatments post-procedure to help minimize the scar. - Infection: This is rare from a biopsy. Post-care is typically band-aids and Vaseline. Signs of infection like yellow to green drainage, expanding redness, and bad odor could be signs of infection. If that is the case, we may prescribe a topical antibiotic or even one by mouth. --- ## Photodynamic Therapy (PDT) **Link:** [Photodynamic Therapy (PDT)](https://www.oceandrivedermatology.com/skin-cancer/photodynamic-therapy-pdt/) ([md](https://www.oceandrivedermatology.com/skin-cancer/photodynamic-therapy-pdt/index.html.md)) **Type:** Service **Description:** Blue Light, photodynamic therapy, or PDT, is a treatment that helps remove premalignant or precancerous lesions on the body. ### Content #### Peace of Mind *Outpatient Photodynamic Therapy (PDT) IN VERO BEACH, FLORIDA | OCEAN DRIVE DERMATOLOGY* As we age, the consequences of our indestructible youth are revealed on our bodies in various ways. One of the most common and worrying is the growth and change in the texture of freckles and small moles on our sun-exposed skin. The good news is pre-cancerous lesions and early-stage skin cancer is highly curable. **With annual cancer screenings, the medical team at Ocean Drive® Dermatology in Vero Beach, Florida, can diagnose and treat these lesions early with photodynamic therapy.** #### Is PDT Right for Me? *What is Photodynamic Therapy (PDT)?* "Blue Light" photodynamic therapy, or **PDT**, is a non-invasive outpatient procedure to remove premalignant or precancerous lesions (actinic keratoses or solar keratoses) on the body. These lesions typically appear on sun-exposed areas of the skin and tend to be rough in texture, almost like sandpaper. The good news is that these **precancerous lesions can be treated early with PDT**, so they do not turn into skin cancer, specifically [squamous cell carcinoma](/skin-conditions/squamous-cell-carcinoma/). #### Less Invasive, Quick Recovery *What are the benefits of Photodynamic Therapy?* There are many benefits to treatment with **photodynamic therapy** Vero Beach patients can look forward to at Ocean Drive Dermatology in Vero Beach, Florida. PTD can work as well as surgery and some types of radiation in treating pre-cancerous and cancerous skin lesions. **PDT is less invasive than surgery** and can be done by medical professionals as an outpatient procedure at less cost than other procedures. If necessary, PDT can be done multiple times on the same site of the body, unlike radiation. PDT targets the cells precisely, resulting in less scarring than other treatment options. Finally, *there are little to no long-term effects*. #### Peace of Mind with Ocean Drive Medical Providers *Your Photodynamic Therapy Consultation* The national standard for a dermatology visit is nine minutes. Ocean Drive Dermatology in Vero Beach, Florida, doesn’t believe in a conveyor belt consultation method. All appointments are scheduled for a minimum of 30 minutes, and their personable staff, from the receptionists to the medical professionals, will know who you are and give your concerns the attention they deserve. **Because Ocean Drive Dermatology is partnered with plastic surgeons, no matter the diagnosis or recommended course of treatment, Ocean Drive Dermatology can meet your needs on-site and in real-time.** #### Quick and Easy Treatment *Your Photodynamic Therapy Procedure* After arriving at the Ocean Drive Dermatology office, your face will be cleaned to remove any oil and makeup. Next, a photosensitizing medication is applied to the affected area and left to absorb for a couple of hours. After the drug-to-light interval, a “blue light” is applied to the site to activate the treatment. Light application lasts approximately 16 minutes. Some Vero Beach **photodynamic therapy** patients report a burning sensation in the first couple minutes, with the remainder of the treatment feeling warm. #### Immediate Results *Photodynamic Therapy Results* Data shows that **PDT can kill precancerous cells and even help prevent skin cancer**. However, *PDT is not a cure-all*. It is still possible to develop new precancerous lesions, sometimes in the same area that was treated. Regular check-ups and returning for treatment when new rough spots appear are recommended. #### Protect Your Skin *Photodynamic Therapy Recovery and Aftercare* Your skin will be extremely sensitive to sunlight for 48 hours after treatment. The best practice is to AVOID sunlight for 48 hours. If you must be outside, you must wear sunscreen and a wide-brim hat after treatment. If you are in direct sunlight without a hat (even if you are wearing sunscreen), the heat and light may reactivate the chemical, and the healing process will take much longer. When you leave the office, a prescription medication to help calm the redness of the affected site will be provided to you. Most patients notice their skin is back to normal within seven days, although some patients take longer to heal. A 12-week follow-up appointment will be scheduled to assess the results and retreat the area, if necessary. As always, continuing sun precautions are a must. Wide-brimmed hats, sunscreens, and sun-protective clothing all help prevent future rough spots. #### Photodynamic Therapy (PDT) *Frequently Asked Questions* --- ## ED&C **Link:** [ED&C](https://www.oceandrivedermatology.com/skin-cancer/electrodessication-curettage-edc/) ([md](https://www.oceandrivedermatology.com/skin-cancer/electrodessication-curettage-edc/index.html.md)) **Type:** Service **Description:** ED&C can be used to remove early basal cell and squamous cell carcinomas. One of the benefits of Electrodessication and Curettage is that there are no sutures involved. ### Content #### Understanding ED&C Procedure *What is an ED&C?* An **electrodessication and curettage** or **ED&C** can be used to remove early basal cell and squamous cell carcinomas. We tend to reserve this procedure for superficial basal cell carcinomas and squamous cell carcinoma in-situ, also called Bowen’s disease. This is typically not a procedure for the face. **Usually you will see this procedure done on the chest, back, arms, and legs.** #### ED&C Procedure Overview *What is the ED&C procedure like?* At Ocean Drive Dermatology in Vero Beach **Electrodessication and Curettage** is done in the office in one of our comfortable exam chairs. The entire process will take less than 30 minutes. The area is first prepped, cleaned and then a local anesthetic is injected. The procedure itself utilizes two main tools; a curette and hyfrecator. The curette is roughly the same size as a pencil and has a sharp rounded end. The hyfrecator also looks like a pencil but has a dull pointed tip on the end that allows us to stop any bleeding during the procedure. The procedure includes three passes with the curette and three passes with the hyfrecator. The first pass with the curette will be a vertical pass followed by the hyfrecator. The second pass with the curette will be a horizontal pass followed by the hyfrecator. Finally, the third pass tends to be a diagonal pass with one last use of the hyfrecator. This procedure allows us to *remove the cancerous tissue and create a safety margin* at the same time. #### ED&C Procedure AfterCare *What happens after the procedure is complete?* Afterwards, you will be left with a circular wound that will need to be cared for. The wound will be a little bit bigger and deeper than before the procedure. However, the bleeding will have been stopped before you leave the office. We will apply a band-aid or pressure bandage along with some Vaseline. It is rare for this type of procedure to cause an infection. You will simply care for your wound with band-aids and Vaseline until the wound has healed. You can get the area wet in 24 hours. Most cases the circle scar will heal hypopigmented or lighter than your normal skin color. #### No Sutures Required for ED&C *Do I need to return to have suture removed?* No, one of the **benefits of an ED&C** is that there are no sutures involved. The only follow up you will need is to return for your next scheduled skin cancer screening. Usually in six months. --- ## Surgical Excisions **Link:** [Surgical Excisions](https://www.oceandrivedermatology.com/skin-cancer/excisions/) ([md](https://www.oceandrivedermatology.com/skin-cancer/excisions/index.html.md)) **Type:** Service **Description:** Most skin cancers are removed or cut out via an excision. Contact Ocean Drive Dermatology today to schedule a consultation with our dermatologists. ### Content #### Surgical Excision *What is a skin cancer excision?* Most skin cancers are removed or cut out via an excision. Basal cell carcinomas, squamous cell carcinomas, melanomas, and even Merkel cell carcinomas can all be removed with a **surgical excision**. Surgical excisions allow us to *remove the skin cancer with a safety margin* at the same time. **The margin is very important for all skin cancers to make sure that no abnormal cells have spread beyond the tumor itself.** #### Minimize The Size Of Your Final Scar *How do you know the skin cancer has been removed?* For most excisions, we utilize an onsite pathologist to confirm that the margins are clear before you leave the office. After the skin cancer has been excised the pathologist starts reviewing the tissue. If there happens to be some left over skin cancer, we know exactly where to remove it. This allows us to minimize the size of your final scar. #### The Excision Will Require Sutures *What about the sutures?* We utilize a layered approach when closing your excision. This means that you will have sutures underneath the skin that you do not see. Also, there will be sutures on top of the skin that you do see. The sutures on top will need to be removed by us and the sutures underneath the skin will absorb. A layered closure allows us to put tension on the subcutaneous and dermal layers of the skin. Having the tension on the deeper layers of skin tends to allow us to create better looking scars when healed. #### Excision Procedure for Skin Cancer *What happens during an excision?* - The skin cancer is marked with a surgical marking pen. This is where we will draw an ellipse around the skin cancer. Typically, the length will be about 4 times longer than the width. Most patients are surprised to see how big the excision is going to be when it is drawn out. This is because we need to create a safety margin, as well as make sure the skin comes back together naturally without creating puckered skin. - TIME OUT! This where we stop for a moment and confirm patient name, DOB, procedure, etc. - The area is cleaned with some rubbing alcohol and local anesthesia is injected. You will feel a little stinging sensation during the injection, which quickly fades away. - The nurse will now “prep” the skin and operating area. This includes cleansing the skin with chlorhexidine and creating a clean field around the excision site. - The skin cancer will now be cut out with a scalpel along the marking pen lines that were drawn earlier. The excision extends to the depth of the subcutaneous fat and is only as wide as what was drawn on the skin in step 1. - As soon as the skin cancer is removed the tissue is marked to delineate the 12 o’clock position. - The nurse takes the skin cancer to our pathologist to confirm that the margins are clear. If they are not clear, the pathologist will tell us where the cancer is still present. This is why we delineate the 12 o’clock position. For instance, the pathologist may tell us the cancer is still present at the 4-6 o’clock position, but clear everywhere else. - The excision is closed with layered sutures. This means you will have sutures that are absorbable underneath the skin and sutures on top of the skin that will need to be removed. - Finally, the excision site is cleaned, dressed with Vaseline and a bandage. You will need to keep the area dry for 24 hours and then shower normally. No rivers, lakes, or oceans until the sutures are removed. - We will schedule you a follow up visit to have your sutures removed. #### Follow-up for Skin Cancer Screening *When do I come back for my next skin cancer screening?* Most of the time for basal cell and squamous cell carcinomas your next skin check will be in 6 months. For melanoma, more frequent exams may be necessary. --- ## Skin Cancer Screening **Link:** [Skin Cancer Screening](https://www.oceandrivedermatology.com/skin-cancer/skin-cancer-screening/) ([md](https://www.oceandrivedermatology.com/skin-cancer/skin-cancer-screening/index.html.md)) **Type:** Service **Description:** A skin cancer screening or total body skin exam is when your body is looked over from head to toes and everywhere in between looking for skin cancer. ### Content #### Skin Cancer Screening Overview *What is a screen cancer screening?* A **skin cancer screening** or total body skin exam is when your body is looked over from head to toes and everywhere in between looking for skin cancer. #### Preparing for Skin Cancer Screening *Do I need to prepare for a skin cancer screening?* There are some things you can do to make the experience as valuable as possible. Come prepared with any areas of concern you want to point out. Also, no make-up does make it easier to visualize spots of concern. If you have make-up on it is easier to miss a spot of concern. You are welcome to bring your make-up along and apply after your visit. #### What to Expect During Screening *What happens when the nurse rooms me?* At Ocean Drive Dermatology, we pride ourselves in making sure the patient is comfortable. During your Vero Beach skin cancer screening you will first be offered a cloth gown (not those crinkly paper gowns). You will have a few minutes to undress and put the gown on. If the room happens to be cold or you are a little chilled, we offer warmed blankets in each of our exam rooms. We want to make sure you are comfortable. You will have the option to keep your undergarments on or take them off. Keep in mind that a **total body skin cancer screening** does involve checking between the buttocks, breasts, and pubic area. Remember, there is no need to be embarrassed. To us, it is just skin. We are not passing any judgment and want to provide you the best possible **skin cancer screening** Vero Beach has to offer. #### Skin Cancer Screening Procedure Steps *What happens during the skin cancer screening?* Once you are ready to go, either Nick Woltjen, PA-C or Maxwell Poling, PA-C and a nurse will enter the exam room. Typically, they will talk with you for a few minutes about your skin cancer history, use of sunscreens, whether you have any specific spots of concern, etc. they will typically start by looking at your scalp. Then he will examine the face, ears, and neck. You might see him use a dermatoscope that allows him to visualize a mole better. Next, we move on to the chest. Here we will take one arm out of the gown at a time. You will never be left exposed on the exam chair. Think of this more like how a massage therapist exposes body parts one at a time and keeps everything else covered. Same thing here. After the chest, arms, and abdomen are examined we will move to the legs and feet. **Finally, we will have you flip over, cover you back up, and look at the back, buttocks, and back of the legs. That is all there is to a skin cancer screening. It is quick, easy, and always with your comfort in mind.** #### Skin Biopsy: What to Expect #### Frequency of Skin Cancer Screenings *How often do I need a skin cancer screening?* This is individualized for each patient, but if you have never had a history of skin cancer, **we recommend a yearly visit for a skin cancer screening**. If you have had a recent [basal cell carcinoma](/skin-conditions/basal-cell-carcinoma/) or [squamous cell carcinoma](/skin-conditions/squamous-cell-carcinoma/), we recommend every six months and more frequently for melanoma. --- ## Acne Treatments **Link:** [Acne Treatments](https://www.oceandrivedermatology.com/skin-conditions/acne/) ([md](https://www.oceandrivedermatology.com/skin-conditions/acne/index.html.md)) **Type:** Service **Description:** Acne, pimples, zits, cysts. No matter how you describe it nobody wants to deal with acne. It's more than skin deep. ### Content #### Who gets acne? Acne, pimples, zits, cysts. No matter how you describe it nobody wants to deal with acne. It’s more than skin deep. The psychological effects of **acne** need to be considered in how aggressive acne is treated. It is not uncommon for patients to not want to take pictures with a group of friends or even look at themselves in the mirror. **Acne is not just limited to teenagers**. Many adults well into their 30s and 40s still deal with acne. Although there is no cure for acne there are many, many good treatment options. #### What causes acne? Acne, which can appear anywhere on the body can be caused by hormones, clogged pores, inflammation, and bacteria. Sometimes it’s a combination of all four. Most people are familiar with what acne looks like. You can have a closed comedone, which is called a white head. Even though you shouldn’t, these are the ones people like to pop. Another form is a blackhead, which is a closed comedone. Then you have the red bumps called inflammatory papules, nodules, or cysts. These are the ones that can be tender and take a long time to go away. It’s possible to have all of these at one time or some people are more prone to whiteheads than inflammatory papules or vice versa. Females may notice that their acne gets worse around their menstrual cycle, but remains relatively clear the rest of the month. Another **common cause of acne** is from microtrauma; we call this acne mechanica. Football players that wear shoulder pads may notice they develop acne where the shoulder pads sit. Women may notice acne worsen or develop on their bra strap area. Treatment does not really change for this type of acne, but can be harder to treat because the area is constantly irritated by repeated microtraumas. Does eating junk food cause pimples? We don’t think so. Diet and acne are not clear cut. **There is a correlation between consumption of a lot of dairy and teenage acne**. Whether this is specifically the dairy or its role on the hormones in our body is unclear. It can’t hurt to decrease your dairy intake to see if it makes a difference. In general, if you notice a worsening of acne with certain foods or drinks; avoid them. It’s not uncommon for acne to run in families. If your mom, dad, brother, sister had bad acne you are at greater risk of having acne, too. #### Treatment Options Treatment for acne can include topical medications (Differin, Retin-A, Epiduo, Clindamycin, Aczone), oral medications (Doxycycline, Minocycline, Azithromycin), and even a combination of the two. Usually **acne treatment** requires multiple therapies at one time so don’t be surprised if you are prescribed a cream to use in the morning, a different one for at night, and possibly an oral medication to take once or twice a day. Remember, above we mentioned that acne can be caused by hormones, clogged pores, inflammation, and even bacteria. This is why multiple treatments are needed so we are attacking each of the problems. If your acne is not responding to typical prescription treatments or you present with nodulocystic acne a medication called isotretinoin or [“Accutane”](https://www.aocd.org/page/Accutane) may be required. This is a five or six-month course of treatment, but can-do wonders in treating acne. In regards to females, sometimes oral contraceptives or other treatments affecting hormones may be of benefit; such as spironolactone. #### It might get worse before it gets better. Anytime that you start a new **acne treatment** you may notice things get worse before they get better. Acne does not develop overnight, thus treatments will not work overnight. It typically takes 4 to 6 weeks to start seeing real improvement in your skin. Signs things are improving maybe be less redness, pimples resolve quicker and fewer are appearing. Also, remember less is more with certain topical acne treatments such as Retin-A. You may notice you get red, irritated, peeling skin if you use too much of the medication or use it too frequently. We will not only prescribe you the appropriate acne medications, but we will make sure you know how to use it correctly to get the best outcomes. --- ## Slow Mohs Surgery **Link:** [Slow Mohs Surgery](https://www.oceandrivedermatology.com/skin-cancer/slow-mohs/) ([md](https://www.oceandrivedermatology.com/skin-cancer/slow-mohs/index.html.md)) **Type:** Service **Description:** Slow Mohs surgery differs from other skin cancer treatments in that it permits the immediate microscopic examination of the removed cancerous tissue. ### Content #### Slow Mohs Surgery *Slow Mohs Surgery at Ocean Drive Dermatology* **Mohs surgery** (pronounced *moes*) is the preferred treatment of skin cancer where preserving the surrounding healthy tissue is critical for functional and cosmetic purposes, such as the nose, ears, face, scalp, neck, hands, feet, and genitalia. **Mohs surgery can also be used in cases where there is a recurrence of skin cancer or if the area of skin cancer is large enough.** #### The Truth About Skin Cancers *What is Slow Mohs Surgery* Skin cancers can often be very deceptive, and we do not know how wide or how deep the cancers extend until the end of the surgery. Mohs surgeries begin by taking the area of skin where cancer seems to be present, as well as the skin around the cancer. While the patient waits, an immediate microscopic examination of the removed cancerous tissue is performed. If cancer cells are still present in the skin around the cancer, the surgeon will remove another thin layer of skin. The review/removal process will continue until no cancerous cells remain. With slow **mohs surgery** Vero Beach providers at Ocean Drive Dermatology are able to **remove the least amount of tissue while ensuring you are cancer free**. #### Peace of Mind *What are the benefits of Slow Mohs Surgery?* The two primary benefits of Mohs surgery are its high cure rate and allowing the patient to keep as much healthy skin as possible. In addition, Mohs surgery only removes the cancerous tissue and leaves the normal tissue untouched. Ocean Drive offers this procedure as an outpatient surgery that does not require going under general anesthesia. **Mohs surgery** is used to treat [basal cell carcinoma](/skin-conditions/basal-cell-carcinoma/) (BCC) or [squamous cell carcinoma](/skin-conditions/squamous-cell-carcinoma/) (SCC), especially when it is aggressive or large. It also treats early-stage melanoma and other rare cancers such as DFSP, extramammary Paget’s disease, and Merkel cell carcinoma. #### Trusted Medical Professionals *Your Slow Mohs Surgery Consultation* All initial **mohs surgery consultations** at Ocean Drive Dermatology are a minimum of thirty minutes. The board-certified medical provider will review your medical history, examine the skin cancer, discuss the surgery, answer questions, and review the pre-op and post-op tasks you need to perform. **As a medical dermatology clinic partnered with plastic surgeons, you will leave the consultation knowing that after cancer has been removed, *you will receive the best cosmetic surgical care*.** #### Gold Standard Medical Care *Your Slow Mohs Surgery Procedure* The Mohs procedure itself involves removing the visible portions of the skin cancer, along with a layer of the surrounding skin. In standard **Mohs surgery**, it is the surgeon who then processes and reads the slides of the skin. Instead of acting as both the surgeon and the dermatopathologist (a pathologist who only reads skin specimens), Ocean Drive Dermatology hires a dermatopathologist to bring a staff and their mobile lab unit to the office to process the tissue. This allows us as the surgeon to tend to your defect and focus on giving you the best cosmetic result. Once the initial layer with margins of skin is removed, it is hand-delivered to the dermatopathologist to process, stain, and read the slides. #### Slow Mohs Surgery *Frequently Asked Questions* --- ## Dry Skin **Link:** [Dry Skin](https://www.oceandrivedermatology.com/skin-conditions/dry-skin-xerosis/) ([md](https://www.oceandrivedermatology.com/skin-conditions/dry-skin-xerosis/index.html.md)) **Type:** Service **Description:** Dry skin can be caused by taking long hot showers, not moisturizing, medications, sun exposure, and even the aging process. ### Content Nobody likes **dry skin**. It can even make you itchy. Dry skin can be caused by taking long hot showers, not moisturizing, medications, sun exposure, and even the aging process can be the culprit. Sometimes we notice dry skin is worse in the winter time when the heater kicks on . As we age our skins moisture barrier is broken down or damaged. The moisturize barrier thins out as we age and can be damaged from UV rays throughout our lives. Typically, you will notice dry flaky skin that is scaly. The skin can be dry anywhere you have skin and you may notice certain areas are worse than other. Some items are easy to modify to help with dry skin. Instead of taking hot showers, consider luke-warm; or even consider bathing every other day if you haven’t been sweating and exercising. Don’t forget to moisturize twice a day. Try a gentle soap such as Dove Sensitive (white bar) that helps moisturize your skin. A culprit of **dry skin** that most don’t realize is because of medications. Blood pressure medications such as hydrochlorothiazide, cholesterol medications such as simvastatin, and even topical medications like Retin-A can dry your skin out. If you have **dry skin** Vero Beach patients should know that one of the best ways to combat it is to moisturize it…twice a day! Try finding a moisturizer that is fragrance free and contains ceramides. Ceramides can help replenish your moisture barrier on your skin, which in turn helps lock in the moisture. If turning down the hot water in the shower and moisturizing is not helping fix your dry skin it is time to make an appointment to be seen. We can help you come up with a skin care routine and make recommendations to **solve your dry itchy skin**. --- ## Injectables Fillers Galleries **Link:** [Injectables Fillers Galleries](https://www.oceandrivedermatology.com/gallery/injectables-fillers/) ([md](https://www.oceandrivedermatology.com/gallery/injectables-fillers/index.html.md)) **Type:** Gallery --- ## Laser Treatments Galleries **Link:** [Laser Treatments Galleries](https://www.oceandrivedermatology.com/gallery/laser-treatments/) ([md](https://www.oceandrivedermatology.com/gallery/laser-treatments/index.html.md)) **Type:** Gallery --- ## Medical Dermatology Galleries **Link:** [Medical Dermatology Galleries](https://www.oceandrivedermatology.com/gallery/medical-dermatology/) ([md](https://www.oceandrivedermatology.com/gallery/medical-dermatology/index.html.md)) **Type:** Gallery ---
